ELECTRIC SERVICE QUESTIONAIRE
In order to insure that you get timely assistance and the type of
permit you need, we request that all applicants for electrical service
upgrades, retags, 2nd services, etc. answer the following questions:
1. What is the service for?
2. Are there any structures on the parcel?
3. Will this be for commercial use?
4. Is this a mobile/modular home? If yes, will the service be on
a pole for the mobile? _
5. Is this a 2nd service on the parcel?
6. Have you contacted PG&E?
7. What size (amps) is the service? _
8. Is this a 3 phase service? _
9. Are you doing any other work?
