LPG TANK AND GAS LINE QUESTIONAIRE
In order to insure that you get timely assistance and the type of
permit you need we request that all applicants for LPG tank and
gas line service upgrades, retags, 2nd services, etc. Please answer
the following questions:

2. Is this a mobile/modular home? If yes, will the service be on a pole
