Arrived at the job at 1000 hr for torque testing of expansion anchors
installed in the top of the concrete stairwell wall adjacent to Grid Line III],
between Grid Lines AA and BB. These anchors were inspected earlier (See
Inspector's Daily Report dated -04-09-96) and found to be only finger tight
Contractor was notified of the deficiency and stated he would correct it_
Inspector Virgil Hancock discovered these bolts had not been tightened prior to
covering with sheetrock and required the sheet rock be removed A torque
wrench was used to tighten the anchors to 175 ft -lb. Both expansion anchors at
this location are now tightened to 175 ft -lb of torque. Contractor was
preparing to place concrete for the Sallyport columns and had completed
excavation for the bases. The columns for this structure (TS 6 x 6) are to be
encased in concrete, without baseplates and anchor bolts. Contractor has
elected to over -excavate the column footings to provide a concrete base upon
which to seat the column. At this time, contractor is preparing to place
concrete for the bases. Rebar cages which surround the column have been
lengthened to extend into the deepened base. Although the cages were found to
be somewhat rusty, they were judged to be satisfactory. Excavations were
inspected and found to be firm and free of loose soil Two of the columns are
aligned over a 10" water line anda special detail was prepared by the engineer
to shift the column loads to the sides of the water pipe This detail required

encasing the water pipe with an insulation -packed sleeve and installing a rebar
cage on each side of the pipe. A grade beam is to be constructed to span over
the pipe and be supported by the rebar cages. Initial concrete placement in
this assembly is to be to the level of the bottom of the column. All
preparations for placement of concrete over the water pipe (both locations) was
as shown in the special detail. Departed the job at 1130 hr. due to other work
commitments. ATC employee, Brad Forsythe will arrive at 1200 hr. for special
