Pursuant to section 19827.5 of the California Health.and Safety Code, all
demolition permit applicants are required to fill out this form.
"19827.5. A demolition permit shall not be issued by any city, county,
city and county, or state and local agency which is authorized to issue
demolition permits as to any building or structure except upon the receipt
from the permit applicant of a copy of each written asbestos notification
regarding the building that has been required to be submitted to the United
States Environmental Protection Agency or to a designated state agency, or
both, pursuant to Part 61 of Title 40 of the Code of Federal Regulations,
or the successor to that part. The permit may be issued without the applicant
submitting a copy of the written notification if the applicant declares that.
the notification is not applicable to the scheduled demolition project. The
permitting agency may require the applicant to make the declaration in writing,
or it may incorporate the applicant's response on the demolition permit.appli—
cation."

?Dear Mr. & 'Mrs. Flack:
We sent you a warning letter dated August 4, 1992 notifying you that you
are in violation of the Butte County Code at the above referenced locution.
As of this date, the following violations still exist.
Failure to obtain approval of previous corrections and failure to obtain
final inspection prior to use and permit expiration for electric ser-
vice change for single family residence in violation of the Uniform.
Administrative Dode Provisions for 1987, The national Electric Code
adopted by Section 27-1 of the Butte County Code as follows:
(a) Section 301(x) Permits Required
(b) Section 305(a) Inspections and Approvals Required
The above violation shall be corrected or abated by you applying for a permit
to complete the work and paying the appropriate fees, within thirty (30)
days of the date of this letter. After permit issuance and field author-
ization to ,proceed, the corrections must be completed and approved by this
office within the permit specified time.
Unless the violation(s) is (are) so corrected or abated, a citation shall
be issued to you to appear in court for said violation(s) and for failing;
to comply with this notice. Upon conviction of said violation(s) or for
failing to comply with this notice, penalties shall be imposed and a Notice
of Violation recorded in accordance with Section 41-7 of the :butte County
Code.

This letter is in response to a letter dated Aug. 3, 1992 and
our conversation on Aug 17, 1992 regarding code violation of the
above address.
Eagle Electric was under the impression that the above address
had been finalized. The indication is that the tenant had not
provided access to the property as needed for inspection.
Since doing the work, we have not been paid by the tenant (who
authorized the work and said that they would pay).
The owner of the property, who lives in Arizona, has contacted
us but has indicated he will not pay for the work either.
With the situation being as I have explained we do not feel
that we want to go to the extra expense of pulling another permit.
