If applicant is riot the owner, written authorization by the owner or other proof of agency must be submitted in order for the applicant to legally sign the application. Application shall be considered void if not signed by the owner or legal agent. 2. All items on application shall be filled in as completely as possible. If an item is not applicable, please indicate by the term "N/A" 3. It is important that the applicant supply an accurate description of the location of the proposed project, including the following: a. Assessor's parcel number(s) (from the tax bills or Assessor's maps). b. Street addresses (if available). C. REGULATIONS FOR LARGE'FAMIILY DAY CARE FACILITiES BU17E COUNTY PLANNING DEPAATIVIENT The following conditions shall be applicable to all Large Family Day Care facilities as defined by section 24- 201 42011 of the Butte County Code and located within the unincorporated area of Sutw County. It is 4 he applicants responsibility to insure that these conditions have been or can be met prior to the application for an Administrative Permit. .The applicant shall be responsible for providing, as requested by the County, information or proof of compliance with the conditions. 1. The facility is a single family residence that is the principal residence of the provider and the use is ciearly'incidental and secondary to the use of the property for residential purposes. - 2. Properties proposed for large Family day care shall be located at least 500 feet driving distance from any other Large Family Iday,care property and the granting of the permit shall not result in any residence being bounded on more than one side by a Large'Family day care. 3 Large Family day care homes shall meet the following traffic control measures; a. - In addition to providing the required number of parking spaces for the residential use, one off sireet rparking space for each employee of the facility and one off street parking space/loading area shall be provided. The 'driveway area may be used to meet this requirement. b. Large Family day care homes located on principal or minor arterial roads as designated by the General Plan Circulation Element shall provide drop-off and pink -up areas which prevent, vehicles from backing onto such roads. c. Properties proposed for Large Family day care homes shall have frontagei on and access off of a paved road. Roads that are constructed for the purpose of meeting this requirement shall meet the RS -4C standard as contained in the Butte County Improvement Standards contained In Chanter 20 of the Butte County Code: 4. One sign, not to exceed 3 Square feet, shall be allowed. 5. The applicant shall be licensed by the California Department of Social Services, Community Care licensing for a Large Family' Day Care facility. 6. The applicant shall meet all State Fire Marshai4- regulations pertaining to Large Family day Gare facilities. 7. The applicant must meet all other applicable County and State ordinances, statutes, and regulations. If the proposed Large family Day Care facility does not meet these conditions, a Conditional Use Permit shall be required. An affidavit of compliance with the age requirements of this section shall be recorded in the office of the Recorder prior to issuance of building permits. Said affidavitt shaft include the legal_ description of the lot or parcel and shall constitute,a covenant running with the land, binding upon the original owners and their heirs, successors and assigns, limiting the occupancy of the senior citizen dwelling unit to the conditlons described in this section. (4) The senior citizen dwelling unit shall not be sold as a separate Unit unless a parcel containing the unit is created in compliance with the existing zoning and subdivision ordinances and the resulting density is In conformance with the General Plan. (5) Two off-street parking spaces shall be provided for the senior citizen dwelling unit in addition to the parking spaces required for.the primary dwelling unit.
