We provided special inspection on this project from 06-28-01 through 09-16-02. Listed below are the procedures that were inspected and/or tested along with the applicable standards. Earthwork Provided compaction monitoring and observation for under slab fill and Parking Lot Substrate. Performed laboratory moisture / density compaction curves. All work was done per ASTM 1557, Chapters 18 h 33 of the 1997 Uniform Building Code. Concrete Provided continuous inspection during concrete placement to include reinforcing steel placement and slump tests. Monitored placement operations and consolidation. Prepared compression test samples. Tested concrete compression samples in laboratory per ASTM and UBC standards. Welding All structural welding was inspected by an AWS certified welding inspector. All welds were visually inspected to be in conformance with AWS D1.1 specifications. The project consisted of removing all organic and deletrious material from the building pad site and constructing an engineered fill which meets County of Butte requirements. We performed a site investigation on 6/28/01 and met with the contractor to discuss the operations. At the time of this investigation we expressed our concerns of testing procedures on the rocky materials. Our technicians found the soil contained in excess of 30% + 3/4 inch material. Per ASTM - D1557 -91 section 1.2 and ASTM D4718-87 section 1.3, we cannot proceed with conventional density testing- procedures on this .material. All fill would have to be monitored during construction to verify that adequate moisture conditioning and compactive efforts were achieved. We monitored grading operations on site from 06-28-01 to 07-20-01. We verified that all areas to receive fill were bearing on firm sable ground prior to any fill placement. The bottom "toe" of the slope was cut back and "benched" prior to fill operations. All engineered fill was moisture conditioned, placed in 8"-12" loose lifts and compacted with a sheepsfoot compactor. We were on site for much of the fill operations and were satisfied with the compactive effort throughout the import fill. . Based on our experience and witnessing much of the earthwork operations, we' certify per Article 3, sections 6735.5 and 6735.6a of the Business and professions Code that the engineered fill was properly moisture conditioned and compacted . to at least 90% . relative compaction. Applied Testing Consultants is not a licensed surveyor. We do not verify or certify grades or elevations. Test elevations are derived from information provided by the contractor and/or the client. Applied Testing Consultants is not the foundation design engineer for this project. During the course of the conference all of the labor standards and wage requirements which are applicable to the construction work to be performed were discussed in full. These standards and requirements are contained in the following publications: U.S. Department of Labor Regulations, Parts 1, 3 and 5; Labor Standards Administration and Enforcement Guide, State and Federal labor standards provisions; State CDBG Grant Manual (October 2000, www.hcd.ca. og v�g/gmm), and the attached material. HCD will not analyze the surveyor payroll for prevailing wage, but will check to make sure payroll records were received at the County. The County noted that a pre -engineering meeting was held with PG&E last week to determine the service locations. Once the locations are set they will be issued to Trent Construction. The County also indicated that the trenching for the utilities would be done as a change order to the contractor. No 1-5 The County noted that there were two light poles that No would need to be removed by the Contractor- The City of Oroville will pay for this change. 1-6 Trent Construction requested the specifications for the Kitchen Equipment that will be used so that the connections could be sized correctly. SOUTHSIDE OROVILLE COMMUNITY CENTER PROJECT This is an Agreement, dated June 18, 2001 by and between Applied Testing Consultants, Inc., hereinafter referred to as "Consultant" and the County of Butte, a political subdivision of the State of California, hereinafter referred to as "County". WHEREAS, the County of Butte desires to engage Consultant to provide services in connection with construction of the Southside Oroville Community Center, hereinafter referred to as the "Project"; and WHEREAS, Consultant represents that it is licensed, fully qualified, and willing to perform the services required hereunder. NOW, THEREFORE for and in consideration of the covenants and conditions hereinafter set forth, the parties do mutually agree as follows: 1. SCOPE OF WORK. Pursuant to the provisions of Government Code Section 31000, County retains Consultant to perform, and Consultant agrees to perform various special inspection/facilities management representative services as described in Exhibit A, attached hereto and made a part hereof. Consultant represents and warrants that all such services shall be performed either in its offices, on site at 2959 Lower Wyandotte Road, Oroville, or at any other meeting site agreed upon by Consultant and County. To the extent any conflict arises between Exhibit A and the terms and provisions in Sections 1 through 18, the latter shall govern. 2. SCHEDULE, TIME OF PERFORMANCE, AND TERM. Consultant shall work on an "as needed" basis for the services described in Section 1 and shall work diligently in good faith until those services are complete. Consultant shall not be considered in default because of delays in performance due to any cause that is beyond its reasonable control. Consultant shall perform such services when requested by County. Consultant shall begin services within five (5) calendar days after requested by County. The term of this Agreement shall be ten (10) months from the time services commence in accordance with this Section 2. 3. STANDARD OF PERFORMANCE. Consultant represents and warrants that it is professionally qualified to perform the various special inspection/facilities management representative services described in Section 1, acknowledges that County is relying upon Consultant's qualifications to perform these services in a professional manner, and agrees that County's full or partial acceptance of any work does not release Consultant from its obligation to perform the services in accordance with this Agreement unless County expressly agrees otherwise in writing. Consultant shall not be considered to be in default because of any non- performance caused by occurrences beyond its reasonable control; the compensation specified in Section 4 shall be reduced to account for such non-performance. 4. COMPENSATION. County shall pay consultant on an hourly basis as compensation for the services described in Section 1, in accordance with the rates set forth in Exhibit A. The total amount of compensation paid to Consultant under this Agreement shall not exceed $13,000.00 in total unless County and Consultant agree otherwise in writing. Consultant shall submit invoices to County at monthly intervals, showing actual labor hours and services provided. Within 30 days after receiving an invoice, County shall either (a) pay Consultant the amounts invoiced or (b) notify Consultant in writing that County finds the invoice to be improper, specifying the grounds for such finding. If County notifies Consultant that an invoice is improper, then County and Consultant shall meet within ten (10) days after Consultant receives the notice and shall attempt in good faith to resolve their differences. 5. INDEMNIFICATION. Consultant shall indemnify, defend, protect, and hold harmless County and its officers, employees, and agents against all liabilities, claims, demands, damages, and costs (including attorney's fees and litigation costs) to the extent caused by the negligent or intentional acts or omissions of Consultant or its officers, employees, or agents. Consultant's obligation under this Section covers but is not limited to liabilities, claims, demands, damages, and costs arising from injury to, or death of any person (including Consultant's and County's officers, employees or agents) and from damage to, or destruction of, any property (including Consultant's and County's real and personal property). Consultant is not obligated, however, to the extent that the negligent or intentional acts or omissions of County or its officers, employees and agents cause such liabilities, claims, demands, damages, or costs. Consultant's obligation under this Section 5 shall survive termination of this agreement. 6. INSURANCE. During the term of this agreement, Consultant shall maintain insurance policies at its sole cost as set forth in Exhibit B, which is attached to, and made a part of this Agreement.. Before beginning the work described in Section 1, Consultant shall submit the following documents to the Butte County General Services Director: A certificate or certificates of insurance showing that the required policies are in force and, if specifically requested by the Butte County Risk Manager or General Services Director, a copy of each policy, including all endorsements. Consultant's obligation to maintain insurance under this Section is independent of, and shall not limit its obligation to indemnify under Section 5. 7. OWNERSHIP AND PUBLICATION OF DOCUMENTS. All reports, records, technical data, evaluations, calculations, plans, details, estimates, and other work of Consultant under this Agreement (a) shall become County's property and be delivered to County when this Agreement is terminated, and (b) shall not be made available to any individual or organization without County's prior written consent, except as may be required by law. 8. ASSIGNMENT AND SUBCONTRACTING. Consultant shall not assign this Agreement or subcontract performance of any services required under this Agreement without County's prior written consent, which County may withhold for any reason at its sole discretion. 9. INDEPENDENT CONTRACTOR. Consultant is an independent contractor, not an employee of County, and thus is subject to the direction and control of County only for the final result. Consultant shall be solely responsible for paying all required payroll taxes and employer obligations related to its performance under this Agreement, including, but not limited to workers' compensation, liability insurance, social security taxes (i.e. FICA), and tax withholding. This Agreement does not entitle Consultant or its officers, employees, or agents to vacation pay, sick leave, retirement benefits, disability or unemployment insurance, or employment benefits of any kind from County. 10. TERMINATION OF AGREEMENT. Unless. sooner terminated in accordance with this Section, this Agreement shall terminate when County has accepted the services described in Section 1 and Consultant has been compensated in accordance with Section 4. This Agreement may be sooner terminated as follows: (a) County and Consultant may terminate this Agreement at any time by mutual consent; and (b) County may terminate this Agreement at any time, for any reason, by giving Consultant written notice at least fifteen days before the effective date of termination. County shall pay Consultant in accordance with Section 4 for all work completed satisfactorily before the termination date. 11. COMPLIANCE WITH LAW. Consultant shall comply with all applicable federal, State, and local statutes, ordinances, regulation, rules, and order, including but not limited to those concerning equal opportunity and non-discrimination. 12. JURISDICTION. Any litigation concerning this Agreement shall be brought in the Consolidated Courts of Butte County. 13. WAIVER. A party's failure to insist on strict performance of this Agreement or to exercise any right or remedy upon breach of this Agreement shall not constitute a waiver of such performance, right, or remedy. No waiver is binding unless set forth in writing signed by the waiving party. 14. AUDIT. For a period of three (3) years after final payment to Consultant, this Agreement shall be subject to the examination and audit of the State Auditor, whether at County's request or as part of any audit of County, Consultant shall retain copies of all documents and records pertinent to this Agreement for this entire three (3) year period. 15. NOTICES. The County reserves the right to terminate this Agreement upon 15 days written notice to Consultant. Consultant may submit written request to terminate this Agreement only if the County should substantially fail to perform its responsibilities as provided herein. The County will provide written notification of reason(s) for termination and provide Consultant a 15 day period to correct problems. Consultant must resolve all problems to the satisfaction of the County within the 15 day period to prevent termination of this Agreement. 18. INTEGRATION. This Agreement sets forth the parties' entire understanding regarding the matters set forth in Sections 1 through 18. Balanced piping is not necessary because a liquid has no difficulty in turning corners or changing directions. KIDDE nozzles come equipped with permanent predetermined orifices. This means that the liquid will be delivered in the exact quantities necessary to the duct, plenum and appliance hazards as required. All pipe shall be schedule 40 (standard weight) black steel. Pipe may be chrome plated. Galvanized pipe shall not be used. All pipe and fittings must be made tight without pipe dope or thread sealant. Pipe fittings shall be standard weight steel, cast iron. malleable iron or ductile iron. Galvanized fittings shall not be used. Branch line connection and individual nozzle connections may be made by using either the outlet or the run of a tee. GENERAL RULES Stainless Steel Tubing and Fittings Stainless steel tubing may be used on all KIDDE sys- tems. Fittings may be stainless steel compression or stainless steel flare types. Bending of tubing using man- drels is permissible. - Pipe Sizing Pipe sizes are determined by the total number of flow numbers running through a particular piece of pipe. This is r, the flow demand fo(that portion of the system. NOTE: It is not permissible to drop pipe diameters below the required value. For example, using 1/20 (1.3 cm) pipe to flow 18 flow numbers is not permissible. However, increasing pipe diameter is acceptable- (i.e., using 1° (2.5 cm) pipe to flow 18 flow numbers) providing the system complies with intemal pipe volume limitations. Table 3-5. KRS-50 Control Box, P/N 87-120010-001 The KRS-50 Control Box (shown in Figure 2-10) is composed of a CO2 cartridge, spring mechanism and a discharge lever which discharges the CO2 cartridge when: (a) a fusible link actuates in any detector or (b) the Remote Manual Control is operated. The valve, with the CO2 cartridge, is connected to a Pressure Operated Actuator mounted on the system cylinder by 1/4" (.6 cm) O.D. copper tubing using compression or flare fittings. (The tubing is standard, commercially available tubing and is not supplied with the system.) When the CO2 cartridge discharges, the CO2 gas pressurizes the 1/4" (.6 cm) O.D. tubing to the Pneumatic Control Head(s), located on the system Cylinder Valve, and actuates the cylinder(s). The KRS-50 Control Box can operate a maximum of five cylinders, regardless of size. A Pressure Operated Actuator (87-100012-001) is required on each system cylinder, one is supplied with the KRS-50 Control Box. One 1/2" (1.3 cm) Liquid Seal Adapter, 3/8" (1.0 cm) Liquid Seal Adapter and one vent plug is supplied with each KRS-50 Control Box. The KRS-50 Control Box can be mounted in any orientation. Refer to Installation Section of this manual for installation limitations N 2-9 Pressure Operated Actuator Pressure Operated Actuator The Pressure Operated Actuator is used only with the KRS- 50 Control Box. It contains a piston that is driven down by the CO2 pressure when the KRS-50 Control Box is activated. The piston will remain in the "discharged"position as long as CO2 pressure is maintained. Pressure can be relieved from the Pressure Operated Actuator by depressing the schradder Wet Chemical Instruction Manual valve on the top of the Pressure Operated Actuator. This must be done prior to removing the Pressure Operated Actuator from the cylinder valve. A maximum of 5 Pressure Operated Actuators can be used with a single KRS-50 Control Box. If the temperature rise is great enough, the detector contacts will close at a temperature somewhat below the set point. Each is used as a means to actuate the system manually from a remote location. This is accomplished by removing the pull pin, releasing the tension in the detection cable, allowing the Control Box to activate the cylinder. If an End -of -Line Remote Manual Control is not applicable, it can be converted to an In -Line Remote Manual Control by using an In -Line Kit. Refer to the installation section of this manual. An In -Line Kit uses a Tee Pulley and Comer Pulley Bearing (see Figure 2-21) to modify the End -of -Line Remota Manual Control. CABLE CONNECTION When cable connection is made, be sure there is no binding or kinking of the cable that would prevent the valve from tripping closed. Balanced piping is not necessary because a liquid has no difficulty in turning comers or changing directions. KIDDE nozzles come equipped with permanent predetermined orifices. This means that the liquid .will be delivered in the exact quantities necessary to the duct, plenum and appliance hazards as required. All pipe shall be schedule 40 (standard weight) black steel. Pipe may be chrome plated. Galvanized pipe shall not be used. All pipe and fittings must be made tight without pipe dope or thread sealant. Pipe fittings shall be standard weight steel, cast iron. malleable iron or ductile iron. Galvanized fittings shall not be used. Branch line connection and individual nozzle connections may be made by using either the outlet or the run of a tee. GENERAL RULES Stainless Steel Tubing and Fittings Stainless steel tubing may be used on all KIDDE sys- tems. Fittings may be stainless steel compression or stainless steel flare types. Bending of tubing using man- drels Is permissible. Pipe Sizing Pipe sizes are determined by the total number of flow numbers running through a particular piece of pipe. This is the flow demand for, (that portion of the system. NOTE: It is not permissible to drop pipe diameters below the required value. For example, using 1/2' (1.3 cm) pipe to flow 18 flow numbers is not permissible. However, increasing pipe diameter is acceptable (i.e., using 1' (2.5 cm) pipe to flow 18 flow numbers) providing the system complies with internal pipe volume limitations. KRS-50 Control Box, P/N.87-120010-001 The KRS-50 Control Box (shown in Figure 2-10) is composed of a CO2 cartridge, spring mechanism and a discharge lever which discharges the CO2 cartridge when: (a) a fusible link actuates in any detector or (b) the Remote Manual Control is operated. The valve, with the CO2 cartridge, is connected to a Pressure Operated Actuator mounted on the system cylinder by 1/4" (.6 cm) O.D. copper tubing using compression or flare fittings. (The tubing is standard, commercially available tubing and is not supplied with the system.) When the CO2 cartridge discharges, the CO2 gas pressurizes the 1/4" (.6 cm) O.D. tubing to the Pneumatic Control Head(s), located on the system Cylinder Valve, and actuates the cylinder(s). The KRS-50 Control Box can operate a maximum of five cylinders, regardless of size. A Pressure Operated Actuator (87-100012-001) is required on each system cylinder, one is supplied with the KRS-50 Control Box. One 1/2" (1.3 cm) Liquid Seal Adapter, 3/8" (1.0 cm) Liquid Seal Adapter and one vent plug is supplied with each KRS-50 Control Box. The KRS-50 Control Box can be mounted in any orientation. Thermostats are equipped with resettable, normally open contacts which close when a predetermined temperature is reached. Thermostats are constructed to compensate for rate of temperature rise. If the temperature rise is great enough, the detector contacts will close at a temperature somewhat below the set point. Each is used as a means to actuate the system manually from a remote location. This is accomplished by removing the pull pin, releasing the tension in the detection cable, allowing the Control Box to activate the cylinder. If an End -of -Line Remote Manual Control is not applicable, it can be converted to an In -Line Remote Manual Control by using an In -Line Kit. Refer to the installation section of this manual. An In -Line Kit uses a Tee Pulley and Comer Pulley Bearing (see Figure 2-21) to modify the End -of -Line Remotg Manual Control. IMPORTANT: For the protection of the valve, install a strainer or filter suitable for the service involved in the inlet side as dose to the valve as possible. Periodic cleaning Is required, depending on service conditions. See Bulletins 8W, 8601 and 8602 for strainers. CABLE CONNECTION When cable connection is made, be. sure there is no binding or kinking of the cable that would prevent the valve from tripping closed. IMPORTANT: Be sure that the load developed by the attachment mechanism at the valve stem dors not exceed the valve dosing spring force when tripped. The time between deanings will vary depending on medium and service conditions. In general, sluggish valve operation or excessive leakage will indicate that cleaning is required. Clean valve disc and seat with a lint -free cloth. Inspect seat in valve for nicks or damage that would cause seat leakage. On 1/2 and 3/4" sizes, lubricate valve disc with DOW CORNING" III compound or an equivalent high-grade silicone grease. On all sizes, lubricate body gasket with the same lubricant. 3. Reassemble in reverse order of disassembly paying careful attention to exploded view provided for identification and placement of pans. 4. Replace bonnet screws, tightening them in a crisscross manner. For 1/2 and 3/4" NPT, torque bonnet screws 110 t 10 inch -pounds 112,4 ± 1,1 newton meters]. For 1, 1-1/4, 1-1/2 and 2" NPT valves, torque bonnet screws 144 + 15 inch -pounds [16,3 ± 1.7 newton meters). For 2-1/2 and 3" NPT valves, torque bonnet screws 250 :t: 25 inch -pounds [28,3 t 2,8 newton meters).
