NOTICE TO APPLICANT

If applicant is other than owner, a proof of agency must be supplied in writing,
in order to legally sign the "Application for Use Permit/Variance".

Application shall be considered void if not signed by owner or his legal agent.

All items on application shall be completed, If an item is not applicable, indl;
tate by the symbol n/a.

Pxplain briefly yet completely the nature of the proposed Use-variance. This
is very important and a requisite,

Ten copies of a detailed plot plan folded 81/1" X111" shall accompany and be
made a part of the "Application for Use Permit/Variance'
The plot plan shall include the following;
a. A scaled drawing of the parcel(s) boundaries.
b.. Lor ,tion of all existing and proposed buildings and improvements.
c. Lucation and size of existing and proposed access to the subject parcel(s),
d, identify streets bordering property and nearby crossroads. Provide a loca-
tion map if necessary. Show North Arrow,

Applicant shall further understand that the "Application for Use Permit_
Variance" shall be subject to the approval of the Planning Commission, or,
upon appeal, of the Board of Supervisors, Any subsequent special conditions
ordered by the Planning Commission shall be made a part of the approved
"Use Permit/Variance" and shall be 'binding on the applicant,

Unless conditions of approval have been complied with, in a one-year period',
the "Use Permit" will be deemed expired.

Proponents: Mr. Merrill Burt - Feels that this area is in need of a
veterinary hospital. Has Epoken with neighbnrs (2 closest)' and there
is no objection. He is agreeable with conditions of approval. He!,
specialiates in small animals.
Mr. Gerald Will - Spoke in favor of the use permit.
commissioner Moore questioned the noise problem and the boarding of
animals. Would applicant be agreeable to conditioning that there be
no general.. boarding, with the exception of sick animals and that
there be no outside runs. Mr. Burt was agreeable.
Commissioner Watson made a motion to approve this use permit with
the stipulation that there be no general boa:tding--strictly for

treatment of sick animals, finding tit in conformance withhat i I
the General Plan, no contact with a concentration of people, and would
not be detrimental to the environment. commissioner Thebach seconded
the motion. Also, subject tb conditions recommended by staff.

A motion was made by Commissioner Hanford, seconded by Commissioner

proposal unless there are neigllibor complaints in the future. The Committee
reviewed the Checklist and a YES was marked on Nos. 2, 11`, & 22, Mr. F''lanery
made a motion to file a Negative Declaration because of failure to find
significant adverse environmental impact. He desired noted that the Com-
mission and Board should refer to the Johnson, Skouris & Dalton EIR since
most of the potentiali i'mpactsof this project have been addressed in this
EIR. Mr. Toussaint seconded this motion.
