We recommend that Simpson PHD5 be used with 5!8" threaded rod embedded 12" with Simpson SET Epoxy. According to Simpson catalog C-2001, effective 11112000, the capacity of a 5i8" diameter epoxy anchor is 6680 lbs., which is greater than the design value for the PHD5. We were present during the installation of all pins. Three of the four anchors were installed properly and in accordance with our recommendations. One of the anchors was not installed during the inspection. When the hole was being drilled for the anchor bolt on the west end of the east wall at the garage, a piece of reinforcing bar was hit with the drill bit causing the stem wall to "blow out". A hole, about 5" by 5", was created on the outside face of the stem wall making it impossible to epoxy an anchor bolt. Simpson PAHD42's and HPAHD22's were originally specified per the manufacture recommendations as shown on. plan. These holddowns were installed per Butte County's "Alternate Braced>Wall'iPanel''�detail. During cons-tniCtion the straps were bent 90 degrees in order to construct the walls. In doing so, concrete spalled off which reduces the load capacity of the holddown. Our office recommends that the holddovms be cut off and that concrete patched. New holddowns need to be installed at the ends of the "Alternate Braced Wall Panels". We recommend that Simpson PHD5 be used with 5/8" threaded rod embedded 12" with Simpson SET Epoxy, According to Simpson catalog C-200.0, effective 111%2000, the capacity of a 518" diameter epoxy anchor is 6680 lbs., which is greater than the design value for the PHD5. This holddown configuration has a greater allowable uplift load than either the PAHD42's or ffie HPA.HD72's: Please let us know if this substitution is in conflict with the design of the. watt. The purpose of the investigation .was to witness that the epoxy anchor bolts were installed properly and in accordance with the recommendations given in our previous letter. Vire were present during the .installation of all pins. Three of the four anchors were installed properly and in accordance with our recommendations. One of the anchors was not installed during the .inspection. When the hole was being drilled for the anchor bolt on the west end of the cast -Nall at the garage, a piece of rebar was bit with the shill bit causing the stem wall to "blow out". A hole, about 5" by 5", was created on the outside face of the' stem wall making it impossible to epoxy an anchor bolt.f It was our recommendation that"the',iontractor contact the project engineer f, ;Yana architect in order to find a solution.
