With reference to the above subject, our records indicate that your Building
Permit will expire on the above date. Building permits are valid for one year
and should construction be started but not completed by the expiration date of
the permit, the permit shall be renewed for 1/2 the original Building Permit
Fee (plus a $10:00 "Filing Fee"). The renewal permit will extend the Building
Permit for an additional year from the original expiration date.
Should you not renew your permit in a timely manner, it cannot be renewed and
all work must cease until a new building permit is issued.

CONTROL SERVICE AGREEMENT
The undersigned Terminix affiliate, hereinafter called
"Terminix", hereby agrees to:
1. Provide •a strural pest co o inspection report,

ing, only if indicated by the word "yes," with regard to

3. Provide control service (optional with purchaser) as set
forth' in paragraph 2 above from year to year for the
lifetime of the property by the payment of an extention

expiration date of the initial term of 'this Agreement or
the expiration date of annual extensions. However,
Terminix reserves the right to increase the annual ex-
tension.charge for the fifth or any later extension year
by giving advance written notice to customer and
reserves the further right to adjust the annual extension
charge at any time to cover any alterations in or addi-
tions to the property after the execution of this Agree-
ment or any addition to the obligations of Terminix
under this Agreement resulting from a change in the
' laws of this State or rules and regulations of any agency
thereof. Purchaser reserves the right to terminate this
Agreement as of any expiration date. As long as this
Agreement remains in force by reason of purchaser's
extensions, Terminix agrees to provide inspection ser-
vice at least once annually and that future treatment
found necessary shall be performed at no additional
cost to purchaser. The service included in the extension
of this Agreement also includes the following only if in-
dicated by the word "yes".
If new damage to the structure or contents,
P,5 resulting from future infestations of insects
covered as specified in paragraph 2 above, oc-
curs during the contract term, Terminix will upon
notification and inspection, arrange for necessary
repairs and pay the cost of labor and materials thereof.
New damage is that damage done by insects, specified
in paragraph 2 above, over and above existing
damage at the time this contract was originally entered
into, with live, insects being found in the damaged
area. Terminix's liability for repair to the protected
structure shall not exceed $100,000.
EXCEPTIONS: This Agreement does not cover the
following:
A. Damage resulting from moisture conditions, in-
cluding but not limited to fungus damage.
B. Damage resulting from masonry failure or grade :-
alteration. s`
' C. Drywood termite damage to furnishings or contents.
D. Terminix disclaims any liability for special, inciden-
tal, or consequential damages. The guarantees
stated in this agreement are given in lieu of any
other guarantee or warranties, express or implied,
including any warranty of merchantability or fitness
for a particular purpose.

4. EXISTING CONDITIONS. It is agreed that under this
agreement, Terminix is not responsible for the comple-
tion of any recommendations contained in the struc-
tural pest control inspection report of paragraph 1
above, unless such recommendations are listed as hav-
ing been completed on the completion notice filed in
conjunction with said inspection report. %
5. OWNERSHIP CHANGE. Upon transfer of ownership of
the identified property, protection against covered in-
sect damage can be continued upon the request of the
new owner and upon the payment of a contract transfer
fee of $-1.21C. In the event the new owner fails .to
request co tinued coverage, this Control Service
Agreement will terminate automatically as of the date
of change of ownership.

7. Terminix agrees to use reasonable care in the perfor-
mance of its work, but assumes no responsibility for
damage to any heating pipes, wiring, roof tiles, or
other facilities or appurtenances, plants and shrubs, or
for injuries or death of pets or animals.
8. Any retreatment for the control of drywood termites, as
required by this Agreement, will include fumigation if
necessary in the sole judgement of Terminix.
9. The service and repairs provided in this Agreement are
guaranteed for a period of one year from date of com-
pletion of initial service, subject to the EXCEPTIONS of
paragraph 3.
10. All terms -of agreement between the parties are con-
tained in this Agreement and no other representations
or statements shall be binding upon the parties.
11. In case of non-payment by purchaser, reasonable at-
