On January 91 ` 2007, a CGI Technical Services Inc. representative verified tensloning of the
ASTM A325 high-strength ,bolts at the referenced project site. Bolts and nuts were
IdentMed per the AISC M,inual of Steel Construction,
Tensioning was by the calibrated wrench method. Our representative calibrated the
erectors tensioning procedures usingthree-boh assemblages for each bolt length and
diameter In our Skidmore Wilhelm ension calibrator,
Based on the calibration tests and our observations, to the beat of our know)cd, the
work was completed in accordance with the approved plans and sp�,ecMtcations. Tensioning
at the observed connections meet the minlmum tension requirements of the Uniform
Building Code.
We have employed generally accepted inspection and testing procedures. However, we do
not undertake the guarantee of construction nor do we relieve the contractor of his
primary responsibility to Rroduce a completed project conforming to the project plans and
specifications. No warranty, either express or Implied Is made.

Within one year from the date of application for a building permit, all other required permits and clearances
from other entities must be obtained for the permit to be issued. Failure to obtain these permits/clearances
will void the application.
Typically other required permits/clearances include, but are not limited to, verification the parcel was
legally created, adherence to. all mitigations and conditions imposed on the parcel at time of creation, as well
as zoning requirements, legal access, and applicable set -backs and environmental issues (fire, agriculture

SPECIAL INSPECIZON NOTE
Special Inspections — Uniform Building Code section 1701: In addition to the inspections
required by Section 108, the owner or the engineer or architect of record acting as the owner's
agent shall employ one or more special inspectors who shall provide inspections. during
construction on the types of work listed under Section 1701.5. The special inspector shall be a
qualified person who shall demonstrate competence, to the satisfaction of the building official,
for inspection of the particular type of construction or operation requiring special inspection.
Duties and Responsibilities of the Special Inspector:
1. The special inspector shall observe the work assigned for conformance with the approved
design, drawings and specifications.
2. The special inspector shall furnish inspection reports to the building official and the engineer
or architect of record. All discrepancies shall be brought to the immediate attention of the
contractor for correction, then, if uncorrected, to the proper design authority and to the.
building official.
3. The special inspector shall submit a final signed report to the.Butte County -Building Division
stating whether the work requiring special inspection was, to the best of his knowledge, in
conformance with the approved plans and specifications and the applicable provisions of this
code.
4. The special inspector shall advise the contractor that Butte County Building Division
inspections cannot be delegated tohim, so inspections must also be made by the Butte
County Building Division
5. Any change in special inspection firms made after permit issuance shall be approved by the
Butte County Building Division prior to the new firm performing any inspections.
6. Special inspections are in addition- to the regular inspections performed by the Butte County
Building Division. Butte County inspection approval and sign off is not to be construed as
authorization to proceed with work which obscures, covers or otherwise prevents proper
special inspection.

SPECIAL INSPECTIONS
I. REQUIRED SPECIAL .IN5PECTION5 SHALL BE PERFORMED BY AN INDEPENDENT
SPECIAL INSPECTOR PER SECTION 1101 OF THE UBC AND LOCAL BUILDING
DEPARTMENT REQUIREMENTS.
2. THE CONTRACTOR SHALL SUBMIT SPECIAL INSPECTOR_ CERTIFICATIONS TO THE
BUILDING DEPARTMENT AT THE TIME OF PERMIT APPLICATION. THE SPECIAL
INSPECTOR MUST BE APPROVED BY THE BUILDING DEPARTMENT PRIOR TO
BEGINNING CONSTRUCTION.
3. ALL IN5PECTOR'S HANDWRITTEN DAILY LOGS SHALL BE AVAILABLE ON THE JOB5ITE
FOR REVIEW BY CITY FIELD INSPECTORS.
d. ALL INSPECTORS REPORTS NOTING DISCREPANCIES ARE TO BE FILED WITH THE
BUILDING DEPARTMENT WEEKLY DURING CONSTRUCTION, WHETHER CORRECTED OR NOT.
5 UPON COMPLETION OF THE PROJECT, THE SPECIAL INSPECTOR SHALL SUBMIT A FINAL
REPORT TO THE ENGINEER AND BUILDING DEPARTMENT STATING THAT ALL REQUIRED
SPECIAL INSPECTIONS WERE COMPLETED, AND THAT, TO THE BEST OF 1415 KNOWLEDGE,
ALL WORK REQUIRING SPECIAL INSPECTION WAS COMPLETED IN CONFORMANCE WITH
THE APPROVED PLANS AND SPECIFICATIONS AND THE APPLICABLE WORKMANSHIP
PROVISIONS OF THE CODE.
6. SPECIAL INSPECTION IS REQUIRED FOR THE FOLLOWING ITEMS:

ALL HIGH STRENGTH BOLTS ARE TO BE INSTALLED USING "TURN OF THE NUT"
METHOD SPECIFIED IN THE NINTH EDITION OF THE AI5C "SPECIFICATION
FOR STRUCTURAL JOINTS U51NG A5TM A325 OR A490 BOLTS".
TESTER SHALL TEST ALL A325 BOLTS AT ALL STRUCTURAL TENSION CONNECTIONS.
CALIBRATE BOLTS AND TENSIONING PROCEDURE. OBSERVE TENSIONS AT ALL
LOCATIONS. IDENTIFY NUTS, BOLTS AND WASHERS FOR HARDNESS AS A5TM
A-325. VERIFY PROPER FIT -UP OF STRUCTURAL STEEL MEMBERS.

A. ALL WELDING NOT PERFORMED IN AN APPROVED SHOP SHALL RECIEVE 1
SPECIAL INSPECTION UNLESS WELDING 15 NOTED A5 'REQUIRED FOR
