NOTICE. TO APPLICANT -USE PERMIT
1. If applicant Is other than owner, a proof of agency must be supplied In writing
in order to legally wgn the "Application for Use Permit.".
Application shall be considered void If riot signed by owner or his legal agent.
2. All Items on application shall be completed. If an Item Is not applicable, indi-
cate by the symbol n/a.
3. Explain briefly yet completely the nature of the proposed use, This is very
important and requisite.
4. Fourteen copies of a detailed plot plan of adequate scale to clearly delinwite
proposed building kind Improvements, folded 8YV' X 11" shall accompany one, be
made part of the "Application for Use Permit"
The plot plan shall Include the following:
a. A scaled drawing of the parcel(s) boundaries,
b. Location of all existing and proposed buildings and Improvements.
c. Location and size of existing and proposed access to the subject parcel(s),
di Identify streets boWering property and nearby crossroads, Provide a loca-
tion map If necessary. Show North Arrow.
e, Preliminary design for on-site, off-sito improvements and storm drainage disposal.

County Department of Public Works for approval and install required facilities,
