Please send or bring in an accurate plot map of the parcel where the tank is and the $65.00 processing fee. Show property lines, structures, driveways, roads, wells, streams/river etc. on the plot map. The destruction of the tank entails pumping, if necessary, breaking up the tank including the lid and making a hole on the bottom, filling with clean soil, sand, or pea gravel and packing the fill to eliminate settling as much as possible. Call t�e Health Department for an inspection when the tank is destroyed, but before the final cover is made so that the inspector can see part of the de- stroyed tank. Final earth cover is made after the Health Department's in- spection. Please mail your plot map and remittance to --the above marked.address.
