Beam # 1 is a 5 l/ 8" x 15" glu-lam beam located over the living room and will be drilled with two '/2" diameter holes located at third points along the beam span. The holes will be centered in the beam and drilled from the top to the bottom. Beam #9 is a 5 1/8" glu-lam located over family room and will be drilled with a '/2" diameter hole at mid span. The hole will be centered in the beam and drilled from the top to the bottom. Item 2 I am approving the installation of 2 - RFB#5x16" bolts to be used with PHD2s at a 4' shear wall panel located on the east wall of the house. I interviewed the contractor, Jim Ratekin, regarding the method used to install the bolts. Jim noted that they were installed with an embedment depth of 10" using a %" diameter drill bit and set epoxy. The uplift force at this location is not large (2.2 kips), so the full capacity of the anchor bolt is not required (7.6 kips at 10" embedment). I am comfortable that he followed the mfg. recommendations and that the installed bolts will be adequate to resist the design uplift force. Concrete a) Concrete shall have a minimum 28 day strength of 2,500 psi u.n.o. b) Concrete shall contain 5 sacks of cement per cubic yard with slump between 3"-5". c) All cement used shall conform to ASTM C-150 and shall be Type II or Type III low alkali. d) Aggregate shall conform to ASTM C-33 and shall not contain materials which are alkali reactive as determined by ASTM C-227, 289, and 295. If test data is unavailable in regards to alkali reactive materials, provide cement with a maximum alkali content less than 0.45% by weight. e) Concrete exposed to freezing or thawing shall be protected in accordance to the latest edition of the ACI code and UBC appendix, chapter 19. f) Concrete shall be air entrained a minimum of 5% and a maximum of 7%. g) Slabs on grade shall be 4" thick and placed over 4" minimum of free draining aggregate base compacted to a minimum of 95% relative compaction. 4" slabs on grade shall be reinforced with #3 bars at 15" on center each way at centerline of slab. h) Saw -cut top 1/4" of slab for crack control at intervals not to exceed 20'. i) Waterproofing of foundations, retaining walls, and slab on grade is the responsibility of the contractor or owner. j) Reinforcement cover shall be as follows: Concrete cast against and permanently exposed to soil: 3" Concrete with soil or weather exposure: #5 bars and smaller 1 '/2" #6 bars and larger 2" Concrete without soil or weather exposure: '/4" k) Reinforcement shall be grade 60 per ASTM A615 u.n.o. Framing/Lumber a) Unless noted otherwise, roof sheathing shall be CDX APA rated sheathing. Thickness shall be per APA load tables based upon roof live load and framing spacing. Apply face grain (long dimension) perpendicular to framing, stagger panels and nail with 8d nails at 6" edge and 12" field. Edge nailing shall apply at all panel edges, over blocking, walls, and drag members. b) Floor sheathing shall be T&G APA rated sheathing. Thickness shall be per APA load tables based upon floor live load and framing spacing. Glue and nail with 8d nails at 6" edge, 10" field, u.n.o. c) Where a wall is labeled as a shear wall, such wall shall be continuously shear paneled and nailed as required per schedule from the roof plywood to foundation sill plate. Where a floor or other element disrupts the continuous paneling, special detailing is required. Where special detailing is not provided, connections shall be in conformance to accepted practice for such connections. d) Where hold downs are required at foundation level of a multi level wall, studs anchored by hold down shall be strapped with Simpson MSTC40 to studs above at any level split U.N.O. Where manufactured "I" joists are specified, Blocking requirements listed herein are to be considered minimums and may be increased as per manufacturers recommendations. i) Unless specifically noted on plans or in calculations, headers shall be 6X10 or 4X12 DF -L (N) #2. Where opening is greater than 5'-0", (2) trimmers shall be used. j) Manufactured "I" joists (such as Truss Joists)shall be installed per the manufactures recommendations using a deflection limit of L/480. Use a manufactured rim board (such as timber strand) with all I joists. Use a double rim board at all locations where ledgers are used (such as deck ledgers). k) Sawn floor joists and roof rafters shall be sized per 1997 UBC Tables 23 -IV -J and 23 -IV -R respectively. 1) Provide double .floor joists under partition walls running parallel to joist span and under all locations where tubs may be located. Adequate support for all other equipment or furnishings including but not limited to: hot water heater, stove, refrigerator, oven, fire place enclosures, wood burning stove, etc... shall be provided by contractor or owner. m) Sill plates shall be pressure treated Douglas Fir or Hem Fir with 5/8" diameter anchor bolts located at 6'-0" o.c. u.n.o. and 1'-0" maximum from end. n) Where anchor bolts have been incorrectly placed, use Hilti Qwik-Bolt of same diameter with 6" embedment in concrete. Install per manufacturers recommendations. o) Splices and joints in double top plate of stud bearing wall shall occur at the center line of supporting stud. p) Top plate splices of stud walls shall be 48" long with (12) 16d sinkers each side of each splice u.n.o. Adjacent walls and beams shall be strapped to top plate with Simpson ST22 u.n.o. q) Fire block stud walls at mid -height or more as required by good construction practice. r) Studs shall be stud grade or better.
