Dear Mr. Greenlee: APN 56-080-143 (Cohasset)
In my letter to you dated march 30, 1994 concerning the above
referenced parcel, item 1(b) required a water sample for
bacteriological potability because the well bentonite annular seal
was left unprotected for so long. After discussing the sample
requirement with your contractor, Mike McDonough, it would appear
reasonable to waive the sample' requirement until the parcel is
actually developed further, has power, and the well is to be put to
use.
Therefore this Department will give final construction approval to
the septic system and the well but will hold any water\sewage
clearance for building construction application until the well has
passed a bacteriological water test(s) taken by this Department.
If you have questions about this matter -please contact me at the
office Monday through Thursday between 8am and 9am.

Sleepy Valley Rd, Cohasset
APN 56-080-143
March 30, 1994 I issued a permit to install a sewage disposal
system on the above referenced parcel. You are hereby notified that
this Department will not give final approval to the septic system
construction or a sewage clearance to the Building Department until
the following have been completed to the satisfaction of the Health
Department.
1. The water well is given a Certificate of Compliance by this
Department.
a) Route the seasonal drain way to by pass the well and
its concrete pad.
b) Provide minimal proof of potable water; A water
sample or samples taken by this Department shows the
water to be bacteriologically potable. Please pay in
advance, $33.00 for this service. (After May 9th fees
will increase.) The Department is requiring a potable
water sample because the well sat for two years with an
unprotected bentonite annular seal. During that time the
seal was exposed -to seasonal drainage water which may
have compromised the integrity of the annular seal. No
concrete pad was installed as required to protect the
well until recently (March 1994).
To take the sample, the Health Department will need an
operating pump, a sealed casing, and a clean faucet with
no leaks.
