This is an application to determine eligibility for fee waivers/reductions of permit fees to rebuild structures
lost and/or damaged in the Butte County Fire Storm — June 2008 (Humboldt and Ophir Fires) when there is
no insurance coverage. This program is time-limited and you must obtain a building permit within 24
months of the event to utilize this fee reduction/waiver opportunity. The following information will be
shared among those County departments working to determine fee waiver eligibility/status and permitting
needs.

DEPARTMENT OF DEVELOPMENT SERVICES Only structures damaged or destroyed in the 2008 Butte
Fire Storm Emergency and constructed with appropriate permits, or constructed prior to building permit
requirements and listed as such in tax assessor's records will be eligible. Should rebuild plans differ from
the permitted structures lost in the fire, owners will be credited the permit fees applicable to the permitted
loss, but will pay the difference for any additional square footage, etc. Structures whose permitted use has
been converted to a non -permitted use after construction will receive fee credit as the type of building that
was actually permitted. The same applies to replacing a mobile home with a stick -built home, credit for the
existing mobile home would be provided toward permit fees for a stick -built home. Agricultural Exempt
permits are no longer valid, and will be permitted as "Limited Agricultural Buildings" as delineated in Butte
County Code. If you do not know permit status, please contact 538-7601 for assistance. Additional
documentation may be required.
PUBLIC HEALTH DEPARTMENT Water supply and sewage disposal systems must be adequate for the
structures they serve.
HAZARDOUS MATERIALS DISPOSAL Must be in accordance with federal, state and local requirements.
Even newer construction materials contain asbestos. The Butte County landfill on Neal Road will notify us
when materials have been disposed of properly. This notification is required to be eligible for fee
waiver/reductions.

Under authority of PRC 4290, the following checked items are required
by the Butte County Fire Department and are made a part of -this
permit. These requirements are minimums and will be superseded by
Butte County local regulations which equal or exceed these standards.
Field inspections will be made by the Butte County Building Department
for compliance.
I�] 1272.00 Maintenance of Defensible Space. To ensure continued
maintenance of properties in conformance with these
standards and measures and to assure continued avail-
ability, access and utilization of the defensible space
provided for in these standards, annual maintenance must
be provided for by the land owner.
Driveway Standards
V] 1273.02 Surface. All driveway surfaces and structures (bridges,
1273.07 culverts and other appurtenant.structures which supple-
ment the roadway bed or shoulders) shall provide unob-
structed access. to-- conventional drive vehicles, includ-
ing sedans and firm ;anp.aratus weighing up to 40,000
pounds.

conductor, have been disconnected.
3. Switch all breakers and switches in the mobilehome to the "on" position.
4. Connect one lead of a test instrument to the mobilehome grounding conductor and
apply the other lead to each mobilehome supply conductor, including neutral.
5. All non-current, carrying metal parts of the mobilehome (aluminum siding, gas line,
water line), including fixtures and appliances, shall be tested for continuity from
such equipment and the grounding conductor.
6. Upon completion of the above procedure, the power supply cord'or feeder assembly
conductors shall be connected to the site service equipment. A further continuity
test shall then be made between the grounding electrode and the chassis of the
mobilehome. Upon satisfactory completion of theelectrical tests, the lot or site
service equipment may be approved for energizing.

For your protection, you should be aware that as "owner -builder" you are the responsible parry oftecord on such
a permit. Building permits are not required to be signed by property owners unless they are personally performing their
own work. If your work is being performed by someone other than yourself, you may protect yourself from possible
liability if that person applies for the proper permit in his or her name.
Contractors are required by law to be licensed and bonded by the State of California and to have a business
license from the city or county. They are also required by law to put their license number on all permits for which they
apply
If you plan to do your own work, with the exception of various trades that you plan to subcontract, you should
be aware of the following information for your benefit and protection:
e If you employ or otherwise engage any persons outer than your immediate family, and the work (including materials
and other costs) is 5300 or more for the entire project, and such persons are not licensed as contractors or
subcontractors, then you may be an employer.
♦ If you are an employer, you must register with the State and Federal Governments as an employer and you are
subject to several obligations including state and federal income tax withholding, federal social security taxes,
workers compensation insurance, disability insurance costs, and unemployment compensation contributions.

This is a courtesy notice to notify, you that you are in violation of the
Butte County Code, as follows, at the -above -referenced location.
Failure to obtain the required .permits, inspections and approvals from
this office for installation of travel trailer and construction of
deck and cabana for mobilehome.
Since permits and inspections are required for the above work; please submit
three (3) complete sets of plans, apply for the required permits, and pay
the appropriate fees. All work must stop until these permits are issued
and you are authorized by our field inspector to proceed. The field
authorization cannot be made until the existing work is inspected and
approved.
It is the County's goal to obtain voluntary compliance with the Butte County
Code. However, you should be advised that Butte County has an active Code
Enforcement Program which provides an effective means of enforcement if
voluntary compliance is not obtained. Enforcement may be pursued through
the issuance of citations, fines and the recording of a Notice of Violation
including a description of the action necessary to abate the violation.
You have thirty 30 days to voluntarily comply with the above directions
or to present an acceptable plan for abatement or corrective actions to
be taken by you. Should you have any questions concerning this matter,
