OWNER -]BUILDER INFORMATION Dear Property Owner: An application for a building permit has been submitted in your name listing yourself as the builder of the property improvements specified. For your protection you should be aware that as "owner -builder" you are the responsible party of record on such a permit Building permits are not required to be signed by property owners unless they are personally performing their own work. If your work is being performed by someone other than yourself, you may protect yourself from possible liability if that person applies for the proper permit in his or her name. Contractors are required by law to be licensed and bonded by the State of California and to have a business license from the city or county. They are also required by law to put their license number on all permits for which they apply. If you plan to do your own work, with the exception of various trades that your plan to subcontract, you should be aware of the following information for your benefit and protection: o If you employ or otherwise engage any persons other than your immediate family, and the work (including materials and other costs) is $200 or more for the entire project and such persons are not licensed as contractors or subcontractors, then you may be an employer. o If you are an employer, you must register with the state and federal government as an employer and you are subject to several obligations including state and federal income tax withholding, federal social security taxes, workers' compensation insurance, disability insurance costs, and unemployment compensation contributions. o There may be financial risks for you if .you do not carry out these obligations, and these risks are especially serious with respect to workers' compensation insurance. o For more specific information about your obligations under federal law, contact the Internal Revenue Service (and, if you wish, the U.S. Small Business Administration). For more specific information about your obligations under state law, contact the Department of Benefit Payments and the Division of Industrial Accidents. If the structure is intended for sale, property owners who are not licensed contractors are allowed to perform their work personally or through their own employees, without a licensed contractor or subcontractor, only under limited conditions. A frequent practice of unlicensed persons professing to be contractor is to secure an "owner -builder" building permit, erroneously implying that the property owner is providing his or her own labor and material personally. Building permits are not required to be signed by property owners unless they are performing their own work personally. Information about licensed contractors may be obtained by contacting the Contractors' State License Board in your community or at 1020 N Street, Sacramento, California 95814. Please complete and return the enclosed owner -builder verification from so that we can confirm that you are aware of these matters. The building permit will not be issued until the verification is returned. Sincerely, * 7 __ AY MRe_ Scott Rutherford Chief Building Inspector NOTE: This Owner -Builder Information is required by Section 19830 of the California Health and Safety Code. 14 SITE PLAN REVIEW APPLICATION Date: 0 AP# Permit Number (if applicable) ()&-o3ey APPLICANT INFORMATION Sc01,IC1/Z_.500Ay Owners Name: ' Owners Address: Telephone No. Site Address Proposed Use: Bin Number o2 Parcel Size: 014N 7 PAR-t-,Wbol,2 AL- c (ico CA ?S%a-o Ce_ H -t F-) f,,eL p4LAA R sidential New Single Family Residential ❑ Single Family -Addition ❑ Single Family Remodel Mobile Home Residential Accessory Permanent Second Dwelling ❑ Temporary Mobile Home (Aunt Minnie) ❑ Temporary Travel Trailer ❑ Multi -family Non-residential ❑ New Commercial ❑ Commercial Addition ❑ Commercial Remodel ❑ New Industrial ❑ Industrial Addition ❑ Industrial Remodel Other ❑ Septic ❑ Well ❑ Agricultural Exempt Building ❑ Agricultural Buffer Form ❑ Other: Brief Explanation/Issue: Zone: V-5 GP: DEVELOPMENT SERVICES INFORMATION (For Staff Use) Approved ❑ Conditionally Approved ❑ Resolve Problems Prior to Approval i ❑ Resolved 50 40114,11 ME. - 1 ALL ITEMS CHECKED APPLY TO THE PROPERTY Parcel Is In: ❑ Williamson Act Minimum Acreage: ❑ Residence can be built per contract ❑ Watershed Protection Overlay Zone SRA - (CDF to determine specific requirements) 100 -Year Flood Plain: • Flood Zone: • Flood Panel No.: Index Date: ❑ Sacramento River Reclamation District (Approval must be obtained from the California Reclamation Board) ❑ Feather River Reclamation District (Approval must be obtained from the California Reclamation Board) ❑ North Chico Specific Plan ❑ Chapman/Mulberry ❑ Cohasset Area Use Requires: ❑ Use Permit ❑ Variance ❑ Agricultural Worker Affidavit ❑ Administrative Permit ❑ Minor Use Permit ❑ Minor Variance Zoning: —N-5- General Plan: &= Applicable Building Setbacks: ❑ Setbacks identified on site Plan. ❑ CDF approval needed for encroachments into SRA setbacks ❑ Use Permit/Minor Use Permit Permit Number: Date of Approval: 2 IL Zoning Code Streets & Highways Fire Prevention Subdivision Map Front 0,0( Side C c Street Rear to Height Waterway N/A N/A N/A ❑ Setbacks identified on site Plan. ❑ CDF approval needed for encroachments into SRA setbacks ❑ Use Permit/Minor Use Permit Permit Number: Date of Approval: 2 IL q Parcel Created By: Deeds: Date of Creation: Legal Access Provided: ❑ No ❑ Yes Deed of Reference: Legal Access Required ❑ No ❑ Yes Parcel Frontage on Publicly Maintained Road: ❑ No ❑ Yes, Road Name: Complies with County Standards for Deed Creation:❑ No ❑ Yes Comments: ❑ Parcel Deemed to be legal ❑ Verify Legal Parcel ❑ Verify Legal Access ❑ Provide Deed of Creation ❑ Obtain a Certificate of Compliance ❑ Obtain a Merger ❑ Obtain a Lot Line Adjustment ❑ Construct road to: ❑ Meet Parcel size required by zone ❑ Meet current Environmental Health Department requirements Subdivision Map/Parcel Man: Map Date of Recording: q/(7 Lot: A Book: No", Page: C21 3 LAND OF NATURAL WEALTH AND BEAUTY PLANNING DIVISION June 1, 2004 :'' DEPARTMENT OF DEVELOPMENT SERVICES 7 COUNTY CENTER DRIVE - OROVILLE, CALIFORNIA 95965-3397 TELEPHONE: (530) 538-7601 Tom Norman FAX: (530) 538-7785 P.O. When fiber rolls are placed at the toe and on the face of slopes, they intercept runoff, reduce its flow velocity, release the runoff as sheet flow, and provide removal of sediment from the runoff. By interrupting the length of a slope, fiber rolls can also reduce erosion. Suitable Applications Fiber rolls may be suitable: ■ Along the toe, top, face, and at grade breaks of exposed and erodible slopes to shorten slope length and spread runoff as sheet flow ■ At the end of a downward slope where it transitions to a steeper slope ■ Along the perimeter of a project ■ As check dams in unlined ditches ■ Down-slope of exposed soil areas ■ Around temporary stockpiles EC Erosion Control J SE Sediment Control ✓ TC Tracking Control WE Wind Erosion Control NS Non-Stormwater Management Control WM Waste Management and Materials Pollution Control Legend: ✓ Primary Objective J Secondary Objective Targeted Constituents Sediment ✓ Nutrients Trash Metals Bacteria Oil and Grease Organics Potential Alternatives SE -1 Silt Fence SE -6 Gravel Bag Berm SE -8 Sandbag Barrier SE -9 Straw Bale Barrier Limitations ■ Fibe; rolls are not effective unless trenched E A S CIA California Stormwater Quality Association January 2003 California Stormwater BMP Handbook 1 of 4 Construction Fiber Dolls SE -5 ■ If fiber rolls are removed, collect and dispose of sediment accumulation, and fill and compact holes, trenches, depressions or any other ground disturbance to blend with adjacent ground. Costs Material costs for fiber rolls range from $2o - $30 per 25 ft roll. Inspection and Maintenance ■ Inspect BMPs prior to forecast rain, daily during extended rain events, after rain events, weekly during the rainy season, and at two-week intervals during the non -rainy season. ■ Repair or replace split, torn, unraveling, or slumping fiber rolls. o If the fiber roll is used as a sediment capture device, or as an erosion control device to maintain sheet flows, sediment that accumulates in the BMP must be periodically removed in order to maintain BMP effectiveness. Sediment should be removed when sediment accumulation reaches one-half the designated sediment storage depth, usually one-half the distance between the top of the fiber roll and the adjacent ground surface. Sediment removed during maintenance may be incorporated into earthwork on the site of disposed at an appropriate location. ■ If fiber rolls are used for erosion control, such as in a mini check dam, sediment removal should not be required as long as the system continues to control the grade. Sediment control BMPs will likely be required in conjunction with this type of application.
