With reference to the above subject and your proposal to install a doughnut shop
in a portion of the building located at 14439 Skyway, the requested inspection
was ihade on February 10, 1983. The inspection revealed the following items which
must be done or resolved:
. (1) Obtain approval from Health Department on sewage disposal system and
also plan approval for conformance to the Restaurant Act requirements.
(2) Obtain approval from Planning Department for off street parking. Submit
a floor and seating plan for entire existing building together with a plot pian
showing existing and proposed parking.
(3) Provide a type one hood, complete with a fire extinguishing system,
over fryer unit (see attached drawing).
(4) The new sink must be properly vented and connected to the building
sewer.
It is now in order for you to submit four complete plans to this office, apply
for the required permits and pay the appropriate fees.

Please post the job card on the job site in a conspicuous location for the inspector to
sign during the various phases of construction -:.and also, have the approved set of plans._
on the site at all times. Inspections will not be made if the job card and approved
plans are not on the job at the time of inspection.
Before construction, please review the approved set of plans and make note of any correc-.
tions made in red. If any of these notes or corrections are not clear to you, please
contact this office -- do not proceed with the.work without making the correction.
The job card must be signed by the inspector before proceeding with• each item listed.
Should he not sign the card, a yellow correction notice will list the corrections to be
made and a call back inspection must be made before going any further. Please allow 24
hours for inspection service.
As a reminder to you, it is illegal to occupy this building or portion of building for
which this permit is issued without approval from this office. On certain occasions, a
temporary occupancy will be permitted. Please do not confuse gas or electrical service
to the building as an occupancy clearance. Before occupancy, all of the "final items"
listed on the job card must be signed by the inspector or special permission given.
Your permit expires one year from date of issuance. If the work has started, but is not
completed and finaled by the expiration date, a renewal permit is required.
Upon completion of the work covered by this permit, please contact this office for final inspection.
