At the time of the site visit fo',:ndation trenches were dug, form board installations were in progress, and some reinforcing steel was in place. Roughly 501/o of the building footprint is covered in native fill, with 50% in cut. Trench depths in the fill area ranged from l'-6" to 6'-1" below the,tcp of the sub -grade. Trench bottoms were based a minimum of 12" into native materiaNheeting code requirements for the structural support of one floor and one roof. This foundation verification is not to be usedl as an approval for a concrete pour or fill material placed within the building footprint Footing trenches must be cleared of all debris and loose material and be properly inspected by county building inspections prior to any concrete pour. Fill material under building areas should be placed as engineered fill and compacted in-place in accordance with industry standards. r If you have any questions or concerns regarding this report, please feel free to call me at (530) 873-7731. High concentrations may cause central nervous system depression, narcosis and unconsciousness. May cause kidney, liver and lung damage. Skin: May cause irritation with redness, itching and pain. Methyl ethyl ketone and cyclohexanone may be absorbed through the skin causing effects similar to those listed under inhalation. Eye: Vapors may cause irritation. Direct contact may cause irritation with redness, stinging and tearing of the eyes. May cause eye damage. Ingestion: Swallowing may cause abdominal pain, nausea, vomiting and diarrhea. Aspiration during swallowing or vomiting can cause chemical pneumonia and lung damage. May cause kidney and liver damage. Chronic Prolonged or repeated overexposure cause dermatitis and damage Toxicity: to the kidney, liver, lungs and central nervous system. Carcinogenicity: None of the components are listed as a carcinogen or suspect carcinogen by NTP, IARC or OSHA. The National Toxicology Program has reported that exposure of mice and rats to Tetrahydrofuran (THF) vapor levels up to 1800 ppm 6 hr/day, 5 days/week for their lifetime caused an increased incidence of kidney tumors in male rats and liver tumors in female mice. The significance of these findings for human health are unclear at this time, and may be related to "species specific" effects. Elevated incidences of tumors in humans have not been reported for THF. Mutagenicity: Cyclohexanone has been positive in bacterial and mammalian assays. Tetrahydrofuran was positive in a bacterial assay. Methyl ethyl ketone is not considered genotoxic based on laboratory studies. Reproductive Methyl ethyl ketone and cyclohexanone have been shown to cause Toxicity: embryofetal toxicity and birth defects in laboratory animals. Tetrahydrofuran has been found to cause adverse developmental effects only when exposure .levels cause other toxic effects to the mother. Rev'd 11/412004 Butte County- Department of Development Services ADMINISTRATION' BUILDING' GIS * PLANNING 7 County Center Drive Oroville, CA 95965 (530) 538.7541 Telephone (530) 538.2140 Facsimile Dear Property Owner: An application for a building permit has been submitted in your name listing yourself as the builder of the property improvements specified: For your protection you should be aware that as "owner -builder" you are the responsible party of record on such a permit. Building permits are not required to be signed by property owners unless they are personally performing their own work. If your work is being performed by someone other than yourself, you may protect yourself from possible liability if that person applies for the proper permit in his or her name. Contractors are required by law to be licensed and bonded by the State of California and to have a business license from the city or county. They are also required by law to put their license number on all permits for which they apply. If you plan to do your own work, with the exception of various trades that your plan to subcontract, you should be aware of the following information for your benefit and protection: o If you employ or otherwise engage any persons other than your immediate family, and the work (including materials and other costs) is $200 or more for the entire project and such persons are not licensed as contractors or subcontractors, then you may be an employer. o If you are an employer, you must register with the state and federal government as an employer and you are.subject to several obligations including state and federal income tax withholding, federal social security taxes, workers' compensationinsurance, disability insurance costs, and unemployment compensation contributions. o There may be financial risks for you if you do not carry out these obligations, and these risks are especially serious with respect to workers' compensation insurance. o For more specific information about your obligations under federal law, contact the Internal Revenue Service (and, if you wish, the U.S. Small Business Administration). For more specific information about your obligations under state law, contact the Department of Benefit Payments and the Division of Industrial Accidents. If the structure is intended for sale, property owners who are not licensed contractors are allowed to perform their work personally or through their own employees, without a licensed contractor or subcontractor, only under limited conditions. A frequent practice of unlicensed persons professing to be contractors is to secure an "owner -builder" building permit, erroneously .implying that the property owner is providing his or her own labor and material personally. Building permits are not required to be signed by property owners unless they are performing their own work personally. Information about licensed contractors may be obtained by contacting the Contractors' State License Board in your community or at 1020 N Street, Sacramento, California 95814. Please complete and return the enclosed owner -builder verification from so that we can confirm that you are aware of these matters. The building permit will not be issued until the verification is returned. UK Mic el C. Vieiri C.B.O. Hauling operations to cease when surface material must be shaped to develop a driveable medium. * No hauling, road construction, or maintenance shall occur when road surface is saturated as defined in CCR 895.1. Emergency road maintenance conducted to alleviate erosion problems does not fall under this limitation. * Erosion control structures will be maintained in working condition during wet weather road use at any time of the year. NOTE: "Winter period" means the period between November 15 and April 1, except as noted under special County Rules at Title 14 CCR 925.1, 926.18, 927.1, and 965.5 (a) except as otherwise provided in the rules: (1) All waterbreaks shall be installed no later than the beginning of the winter period of the current year of timber operations. Saturated Soil Conditions means that site conditions are sufficiently wet that timber operations displace soils in yarding or mechanical site preparation areas or displace road and landing surface materials in amounts sufficient to cause a turbidity increase in drainage facilities that discharge into Class I, 1I, III, or IV waters, or in downstream Class I, II, III, or IV waters that is visible or would violate applicable water quality requirements. In yarding and site preparation areas, this condition may be evidenced by: a) reduced traction by equipment as indicated by spinning or churning of wheels or tracks in excess of normal performance, b) inadequate traction without blading wet soil, c) soil displacement in amounts that cause visible increase in turbidity of the downstream waters in a receiving Class 1, II, III, or IV waters, or in amounts sufficient to cause a turbidity increase in drainage facilities that discharge into Class I, II, III, or IV waters, or d) creation of ruts greater than would be normal following a light rainfall. On logging roads and landing surfaces, this condition may be evidenced by a) reduced traction by equipment as indicated by spinning or churning of wheels or tracks in excess of normal performance, b) inadequate traction without blading wet soil, c) soil displacement in amounts that cause visible increase in turbidity of the downstream waters in receiving Class I, II, II1, or IV waters, or in amounts sufficient to cause a turbidity increase in drainage facilities that discharge into Class I, II, III, or IV waters, d) pumping of road surface materials by traffic, or e) creation of ruts greater than would be created by traffic following normal road watering, which transports surface material to a drainage facility that discharges directly into a watercourse.
