'County procedures, zoning provisions and possible conditions of approval with the Development Services Staff. The following items are required to be submitted at the time of application: "Complete a Pre -Application Review with the Environmental Health Division for on-site -sewage disposal requirements for the proposed use. The Environmental Health Division charges a $46.00 fee for this review. 2. The completed, signed, Uniform Application and Affidavit. If the application is signed by an agent for the property owner, and agent authorization form must be submitted along with the Application. The Application shall not be accepted unless signed by the owner or legal agent. 3. Six (6) copies of a plot plan drawn to'scale. The finished maps shall be folded to 8 '/2" x I V. The plot plan must include: * Name and address of Applicant/Owner. * Property lines and lot dimensions * Assessor Parcel Number(s) and the street address. * Dimensioned locations of existing and proposed dwellings and improvements on the property (including, but not limited to , buildings, driveways, parking areas, wells, septic tanks and leach fields). If the mobile home is removed at the end of the permitted period, the deposit, or surety bond, shall be returned to the applicant upon verification of the mobile home removal. He has decided to not pursue the occupancy permit at this time. Dear Mr. Green, In checking our records to verify that your deposit for your Administrative permit had. been processed, we were informed by the County, Auditors Office, that we could not refund the deposit to you as the receipt shows that the fees were paid by Gabriel Scott Green. If you would refer to receipt #394727, dated 1/27/04, (1 have attached a copy) you will see that it was made out to Gabriel Scott Green. According to Butte County Code, we can only refund the person whose name appears on the original receipt. r In order for us to refund the $2000.00 deposit plus interest, we will need a letter from Gabriel Scott Green requesting that the refund be returned. PERMIT REQUIREMENTS: Approval for a temporary second dwelling is subject to the following requirements: A mobile home certified under the 1974 National Mobile Home Construction and Safety Standards Act. Occupancy of the mobile home shall be limited to Gabriel Scott. An affidavit attesting to the relationship of the involved parties was submitted with the permit application. 2. No rent is to be charged to the occupant of the mobile home. The temporary mobile must meet the requirements of the Butte County Environmental Health Department for domestic water supply and sewage disposal. The parcel is within Section 4, T23N, R2E, MDB&M and is AP# 56-430-024. PROJECT DESCRIPTION The project is to install a 24 by 44 mobile home and associated utilities and facilities on the southwest corner of the parcel. A septic system will be installed and connection made to an existing water supply. GENERAL PROJECT AREA DESCRIPTION The area involved adjoins Handy Lane, a dirt and gravel road which dead ends at a turn around several hundred feet to the east. At this point, Handy Lane serves three parcels, this being the only one developed. Traffic is light. The project area slopes down from the road in a northward direction at approximately six percent for about a hundred feet and then flattens out to less than a two percent slope. The area is non -timberland containing some black oak and assorted brush. Previously, this was used as horse pasture. Soils have been observed to be fairly deep and appear to be a sandy clay loam that resists erosion. There are no noted erosion control problems associated with the parcel. There is some large rock deposits running through the parcel but not within the project area. EROSION HAZARD RATING Considered to be LOW or Very Low. MITIGATIONS It is not felt that any special erosion control measures are required. No significant excavation for location of the mobile or driveway is required. A minimum of two inches of Class 11 Road Base will be placed on' the driveway. The SEC 100 mix contains 401/o Bland Bromegrass, 51/oZorroFescue, l0%Vetch, l5% Rose Clovers, 20% Crimson Clover, and 10% Subterranean Clovers. Seed is to be evenly spread at 1-3# per 1000 square feet or 35-50# per acre. Seeding is best done after there has been sufficient precipitation to moisten the top several inches of soil. Seeding when the soil is dry reduces the viability of the seed and makes the loss of seed due to bird and rodent activity likely. THAING of WORK It is expected that all work will be completed by the beginning of the rainy season. No work shall be done during periods of heavy rainfall and/ or when it may be reasonably expected that there may be sufficient surface runoff to carry sediment more than one -hundred feet. CERTIFICATION The site has been well known to me for a period of six or more years and was visited in the course of preparation of this plan. The description above is accurate and I believe the measures listed are more than adequate to control any sediment created by the construction activities. Costs - Material costs for fiber rolls range from $20 - $30 per, 25 ft roll. Inspection and Maintenance ■ Inspect BMPs prior to forecast rain, daily during extended rain events, after rain events, weekly during the rainy season, and•at two-week intervals during -the. non -rainy season. ■ Repair or replace split, torn, unraveling, or slumping fiber rolls. ■ If the fiber roll is used as a sediment capture device, or as an erosion control device to maintain sheet flows, sediment that accumulates in the BMP must be periodically removed in order to maintain BMP effectiveness. Sediment should be removed when sediment accumulation reaches one-half the designated sediment storage depth, usually one-half the distance between the top of the fiber roll and the adjacent ground surface. Sediment removed during maintenance may be incorporated into earthwork on the site of disposed at an appropriate location. ■ If fiber rolls are used for erosion control, such as in a mini check dam, sediment removal should not be required as long as the system continues to control the grade.
