RESOLiJTION IN RECOGNITION OF ANN POLIVKA
UPON RETIREMENT FROM
BUTTE COUNTY PUBLIC HEALTH DEPARTMENT
WHEREAS, Ann Polivka has amiounced her retirement as of December 30. 2004; and
WHEREAS, Ann Polivka earned her Bachelor of Science in Nursing fi~om Alfred University, Alfred, New
York in 1977 and was awarded a Family Nurse Practitioner certification fi~om SUNY Binghamton, Binghamton,
New York in 1978; and
WHEREAS, Ann Polivka joined the Butte County Department of Public Health as a Family Nurse
Practitioner on April 20, 1993; and
WHEREAS Ann Polivka has provided caring and comprehensive health care to adults and children in our
commiu~ity while employing bilingual skills and a culturally sensitive manner; and
WHEREAS, Ann Polivka has earned the respect and confidence of Public Health managers, co-workers,
and colleagues through her warm personal manner; and
~'VHF,REAS, Ann Polivka has conducted her practice of public health clinical care with the highest level of~
integrity, as an invaluable team player, and a true cornerstone of the Public Health Department.
NOW, THEREFORE, BE IT RESOLVED that the Board of Supervisors wishes to express to Ann
Polivka sincere appreciation for her many years of dedication and service to Butte County, and further, wishes her
well on her retirement.
PASSED AND ADOPTED by the Butte County Board of Supervisors this 14°i day of December 2004 b_y
the following vote:
