RESOLUTION AUTHORIZING CHAIR TO SIGN
CONTRACT AMENDMENT BETWEEN
STATE DEPARTMENT OF MENTAL HEALTH
AND
BUTTE COUNTY DEPARTMENT OF BEHAVIORAL HEALTH
FOR
FY 2004/2005
WHEREAS, Butte County Department of Behavioral Health (BCDBH) requires Butte
County Board of Supervisors' authorization to amend the existing Phase I and Phase II of the
Inpatient and Outpatient Consolidation Managed Care Program Contract with the State
Department of Mental Health; and
WHEREAS, the total revenue to be received for FY 2004/2005 is $2,276,342 (an
increase of $205,427); and
WHEREAS, the term of the agreement shall be July 1, 2004 through June 30, 2005.
NOW, THEREFORE, BE IT RESOLVED that the Butte County Board of Supervisors
authorizes BCDBH to amend their Inpatient and Outpatient Program Contract for Managed care
services.
PASSED AND ADOPTED by the Butte County Board of Supervisors this 1S' day of
March 2005, by the following vote:
AYES: Supervisors Connelly, Dolan, Houx, Josiassen and Chair Yamaguchi
NOES: None
ABSENT: None
NOT VOTING: None
