WHEREAS, the Board of Supervisors authorized the implementation of Targeted Case Management Services (TCM) in
Butte County on June 12, 2007 for the period of July 1, 2007 through June 3D, 2012 with the approval of County Contract
No. 840954 between County of Butte and State Department of Health Services; and
WHEREAS, pursuant to Section 14132.44 of the Welfare and Institutions Code, TCM became a covered Medicaid benefit
consisting of case management services that assist Medi-Cal eligible individuals within the specific targeted groups to
access needed medical, social, educational and other services as outlined by approved TCM services components; and
WHEREAS, County Contract No. 840954 designated the County of Butte as the Local Government Agency (LGA}
responsible for the oversight of TCM in Butte County; and
WHEREAS, the TCM program is administered by the Public Health Department within the County of Butte and the LGA
Coordinator is a representative of the Public Health Department; and
WHEREAS, participation in the TCM program requires the submittal of various documents to the Department of Health
Services that require execution by the LGA Coordinator, including the annual cost report; and
WHEREAS, the Board of Supervisors has the authority to authorize the LGA Coordinator, or designee, to execute
necessary documents on behalf of the LGA.
NOW, THEREFORE, BE IT RESOLVED, that the Board of Supervisors, authorizes the LGA Coordinator, or designee, to
execute documents necessary for the purpose of implementing TCM within Butte County during the period of October 27,
2009 through June 3D, 2012.

vote:

EXECUTE DOCUMENTS ON BEHALF OF THE LGA FOR ACTIVITIES RELATED TO
PROVIDING TARGETED CASE MANAGEMENT (TCM} SERVICES
