' RESOLUTION AUTHORIZING
BUTTE COUNTY DEPARTMENT OF BEHAVIORAL
TO ENTER INTO A
STANDARD AGREEMENT WITW
STATE OF CALFORNIA, DEPARTMENT OF REHABILITATION
FOR
FY 2011 - FY 2012
WHEREAS, Butte County, through its Department of Behavioral Health {BCDBH}, wishes
to enter into an agreement with the State of California, Department of Rehabilitation (DOR) to
provide vocational rehabilitation services to mutual clients receiving services from BCDBH and
DOR; and
WHEREAS, DOR requires that an individual be authorized to sign and execute any and
all documents required by DOR to effectuate the execution of contracts andlor amendments
except to increase the financial liability of County of Butte;
NOW, THEREFORE, BE IT RESOLVED that the Butte County Board of Supervisors
authorizes BCDBH to enter into an agreement with DOR to provide vocational rehabilitation
services to mutual clients receiving services from BCDBH and DOR; and that Anne Robin, MFT,
BCDBH Director, shall be authorized to sign and execute any and all documents required by
DOR to effectuate the execution of contracts andlor amendments except to increase the financial
liability of County of Butte.
PASSED AND ADOPTED by the Butte County Board of Supervisors this 25th day of
October 2091, by the following vote:
AYES: Supervisors Connelly, Wahi, Kirk
