RESOLUTION ATH I I
BUTTE COUNTY DEPARTMENT F BEHAVIORAL HEALTH
TO ENTER INTO B ENT WITH STATE DEPARTMENT OF HEALTH CARE SERVICES
WHEREAS, Butte County, through its Department of Behavioral Health (BCDBH), wishes to enter into an
agreement with the Department of Health Care Services (DHCS); and
WHEREAS, CHCS requires a resolution designating an individual be authorized to sign said agreement;
NOW,THEREFORE, B IT RESOLVED that the Butte County Board of Supervisors authorizes BCDBH to accept
the terms of the above-referenced agreement OHCS and authorizes the Director of BCDBH to sign said agreement along
with all amendments thereto.
PASSED AND ADOPTED by the Butte County Board of Supervisors this 231d day of September 2014, by the
following vote:
AYES: Supervisors Wahl, Kirk, Lambert and Chair Teet r w
NOES: None
ABSENT: Supervisor Connelly
