RESOLUTION AUTHORIZING CHAIR TO SIGN AGREEMENT BETWEEN
STATE DEPARTMENT OF HEALTH CARE SERVICES
AND
BUTTE COUNTY DEPARTMENT OF BEHAVIORAL HEALTH
17-94665,
WHEREAS, Butte County Department of Behavioral Health requiires authorization from the Butte County Board of
Supervisors to enter into the State Department of Health Care Services (DHCS) Strategic Prevention Framework (SPF)
Partnerships for Success(PFS) project agreement for FY 2017-1.8 through FY 2019-20; and
WHEREAS, DHCS requires the Butte County Board of Supervisors to authorize Dorian Kittrell, Director, Butte
County Behavioral Health to sign the SPF PFS agreement and all amendments for FY 2017-18 through FY 2019-20�.
NOW,THEREFORE, BE IT RESOLVED that the Butte County Department of Behavioral Health is authorized by the
Board of Supervisors to accept the SPF PFS agreement with the State Department of Health Care Services for a multi-year
total of$450,00 and to accept above described provisions of this agreement.
PASSED AND ADOPTED by the Butte County Board of Supervisors this 241” day of April, 2018, by the following
vote
AYES:Supervisors Connelly, Wahl, Kirk,Teeter, and Chair Lambert.
