STATE DEPARTMENT OF HEALTH CARE SERVICES
AND
BUTTE COUNTY DEPARTMENT OF BEHAVIORAL HEALTH

WHEREAS, Butte County Department of Behavioral Health requires authorization from the Butte County Beard of
Supervisors to enter into the State Department of Health Care Services (DHCS) Substance Use Disorder Agreement for
Substance Use Prevention and Treatment Block Grants for FY 2017-18 through FY 2019-20; and
WHEREAS, DHCS requires the Butte County Board of Supervisors to authorize Joel Chain, Assistant Director, Butte
County Behavioral Health to sign the agreement and all amendments for FY 2017-18 through FY 2019-20; and
NOW, THEREFORE, BE IT RESOLVED that the Butte County Department of Behavioral Health is authorized by the
Board of Supervisors to accept this agreement with the State Department of Health Care Services for a multi-year total of
$4,921,828 and to accept above described provisions of this agreement,

PASSED AND ADOPTED by the Butte County Board of Supervisors this 261x' day of March, 2019, by the following
AYES: Supervisors Connelly, Lucero, Ritter, Teeter, and Chair Lambert
