LocumTenens.Com, LLC (LocumTenens), is a physician recruiting agency which provides psychiatrists for placement at
commercial and government facilities on a short-term basis. Due to ongoing shortage of psychiatrists, Butte County
Behavioral Health (BCDBH)would like to amend it's agreement with LocumTenens.Com, to provide psychiatrists to work at
BCDBH sites as needed
BCDBH requests approval to amend its agreement with LocumTenens.Com, to add additional service hours and to revise
the start date of the contract for fiscal year 2012-13. The term of the agreement was originally "upon date of execution" of
the agreement. However, BCDBH had an emergency need for a psychiatrist who was placed August 12, 2012, which was
prior to the execution date of the contract, August 28, 2012. The term of the agreement will be August 1, 2012 and will
terminate on June 30, 2013. The maximum financial obligation under this agreement shall not exceed $259,440 (this is an
increase of$100,000).The increase in contract maximum will not change the current contract hourly rate.
Fiscal Impact:
This contract will be funded by State and federal Medi-Cal funds. There is no impact to the County General Fund.
Personnel Impact:
Does not apply.
Action Requested:
Approve amendment and authorize the Chair to sign subject to review by County Counsel and Auditor-Controller.

AMENDMENT A
CONTRACT SERVICES AGREEMENT
BETWEEN
COUNTY OF BUTTE
AND
LOCUMTENENS.COM, LLC
FY 2012/2013

This Amendment is entered into, by and between the County of Butte, a political subdivision of
the State of California, through its Butte County Department of Behavioral Health, hereinafter
referred to as COUNTY, and Locumtenens.com, LLC hereinafter referred to as CONTRACTOR.
1. On August 28, 2012, COUNTY and CONTRACTOR entered into a written agreement entitled
"Contract Services Agreement".
2. The parties now mutually desire to amend said agreement as specified in Paragraph 3 below.
This contract is being amended allowing the CONTRACTOR to provide additional psychiatric
services for FY 12/13.
3. In consideration of the mutual promises contained herein, the parties agree to amend said
agreement as follows:
Page 2, COMPENSATION AND METHOD OF PAYMENT, Revise:
A. County shall reimburse CONTRACTOR for services provided in accordance with Exhibit A for a maximum
compensation of TWO HUNDRED FIFTY-NINE THOUSAND FOUR HUNDRED FORTY DOLLARS ($259,440)
(this is an increase of$100,000 .

A. The term of this Agreement shall be August 1, 2012 through June 30, 2013.
Exhibit A, Fee Structure, revised as attached and incorporated herein per this reference.
4. All other provisions of said agreement shall remain in full force and effect and are hereby re-affirmed
as originally stated.
