serves as notice that Feather River Hospital will no longer be offering Lymphadema
services at the Feather River Outpatient Center at 6283 Clark Road in Paradise, CA,
effective May 20, 2011.

Effective May 20, 2011, the Feather River Hospital
Lymphedema Clinic will be closing at 6283 Clark Road,
Suite 1, Paradise
Lymphedema services will no longer be offered at this
facility. Lymphedema referrals can be directed to:
Enloe Hospital Lymphedema Program, 251 Cohasset
Road, Chico, CA (530} 332-3500. This program accepts
Medi-Cal and Medicare patients.
