RESOLUTION APPROVING AGREEMENT WIT~-I FEATHER RIVER
OPPORTUNITY CENTER AND AUTHORIZING CHAIRMAN T'0 SIGN
WHEREAS, this board wishes to extend psychiatric rehabilitation
services to the citizens of Butte County by agreement with the
Feather River Opportunity Center; and
WHEREAS, said agreement has been prepared and is attached
hereto as Exhibit "A",
NOW, THEREFORE, BE IT RESOLVED by the Board of Supervisors
of the County of Butte, State of California, that this board
approves said agreement attached hereto as Exhibit "A", and the
chairman of this board is hereby authorized to execute said
agreement on behalf of the County of Butte.
PASSED AND ADOPTED by the Board of Supervisors of the
County of Butte, State of California, this 5th day of August,
1969, by the following vote:

THIS AGREEMENT, Wade and entered into this 1 3 day;
°f ____ 1969, by and between the County of Butte
{Mental Health Services), hereinafter referred.to as "County",
and the Feather River Opportunity•Center, hereinafter referred to
as "Center",

WHEREAS, the County desires to expend Dsychiatri.c x`ehabilita• -
ior_ services to the citizens~of Butte County and Center mai:itains
facilities for such; services .at 3141 Richter Avenue, Oroville,
California; and
WFiERE4S, Section 5614 of the Welfare and Institutions Code
of the State of California provides that a mental health service.
may contract with a rehabilitation agency for services,
NOW, THEREFORE, County and Center agree as follo~rs:
FIRST; The terms of this agreement shall cover a'
period from July 1, 19b9, to, June 30, 1970.
SECOiID; Center agrees to provide rehabilitation
services to residents of Butte County as described in-Section S43
of Title 9 of the California Administrative Code as folloZ•rs;
"Psychiatric rehabilitation consists of services
provided u;der.psychiatric direction for a
diagnosed psychiatric condition, to restore,
establish, and/or maintain an optimum of social,
emotional, vocational, and/or physical function-
ing consistent with the limitations imposed by
the sequelae or residuals of mental illness,
mental retardation, or emotional disorder. It
may include, but not be limited to formal
psychaitric treatment."
Such services shall be ender the beneral supervision
nd control of the Director of Mental Health Services of County
nd sha11 permit such director to supervise and specify the kind,

quality and amount of the services and criteria for determininU
the persons to be served. .
THIRD: Services•provided under thi•s•contract shall be
restricted to patients ~•rho are referred to Center by the Director
of t•iental Health Services of County.
FOURTH: Center agrees to furnish said se n-ices to
all patients referred to Center,by the Director of P~Iental Health
Services of County, provided that Center shall not provide more
than S00 patient days of said service.
•FIFTH: Services under this agreement shall not b•e
denied because of inability to pay, or because of race, color,
religion, national orgin or ancestry.
SIXTIi: Contractor agrees to maintain accurate books
and accountir~; records as required by the Director of Mental
Health Services of County. Such books and accounting records
shall be open to inspection by state and local at:ditors at any
reasonable time. Contractor further agrees to maintain statistical
records and provide, on .forms furnished by County, statistical
reports as required by th•e Director of Mental Health Services of
County.
• SEVENTH: County agrees to pay Center for such
services as are provided, at a rate not to exceed $3.00 per
patient day,. In no event shall the fee charged to County exceed
the actual cost of providix~; the service.
EIGH'TT-I: Center shall not be required to furnish
transportation for the patient either to or from Center,
NINTH: Center agrees to carry liability insurance
and to hold harmless County for any injuries sustained by a
patient :•~hile at the rehabilitation facilities of Center.
IN WITNESS WHEREOF, the parties .hereto have executed this
