RESOLUTTON APPROVZNG NURSES TRAINING AGREEMENT AND AUTHORIZ~NG
BIRECTOR OF MENTAL HEALTF3 SERVICES OF THE CQUNTY OF BUTTE
'I`0 EXECUTE ON BEHALF OF THE COUNTY OF BUTTE
NOW, THEREFORE, BE IT RESOLVED that the County of Bu~te
approves the attached torm ot Nurse's Training Agreement to be
entered into between various medical. hospitals, convalescent hospitals
and nuxsing homes and the the County of Butte and hereby authorizes
Gerald E. Maguire, M, D., Director of Mental Health Services of the
County of Butte to execute said Nurses Training Agreement on behalf
of the County of Butte subject to approval for fiscal and budgetary
control by the Butte Coun~y Auditor and further subject to approval
by the State Department of Mental Hygiene.
PASSED AND ADOPTED by the Butte County Board of Super-
visors this 3rd day ot October, 1972, by the following vote:
AYES: 5upervisors Ladd, Madigan and Chairman Gilman
NOES: Supervisors Douthit and Maxon

NTIRSES_TRAINING AGRE~IENT '
THIS AGREEMENT is made and entered a.nto this ~ day of
_^~_^^___~~~~__~,___, 197^^, by and between the County of
Butte, hereinafter referred to as "County'r on behaif of its
Mental Health Services Department and_A_____`~_____~_~__
___J~~~~~______`~^^~_______~~, hereinafter referred to as
"Hospital ".
FIRST: Hospital hereby agrees to provide to County the
services of two nurses who shall be either registered nurses or
licensed vocational nurses for a period of time to be mutually
agxeed upon.
SECOND: During said period of time County, throu.gh its
Mental Health Services Department, wi11 provide training to said
nurses with special emphasis on psychiatric aspects of patient
,care, treating patients with severe emotional problelns and
techniqc~es for changing dysfunctional patient behaviors.
THIRD: County agrees to reimburse Hospital ~or 75% of the
actual cost o£ salaries, exclusive of a11 fringe benefa.ts,
ac~.~a11y paid to said nurses for said period of time.
FOURTH: Hospital shall, each month, submit a claim fi.o
Butte Coun~y Mental Health Services for reimbursement ~or such
salaries which claim sha11 be on a form prescribed by, and complet-
ed in accordance with the claim preparation instructions pxegared
by, the Director of the State Department of Mental Hygiene.
FIFTH: Hospital agrees to t~ai.ntain accurate books and
accurate records as required by the Airector of Mental Health
Services far County. Such books and accounting records shall be

open to inspectiori by State and 1oca1 auditors at any reasonable
~ime. Hospital further agrees to furnish said Director of Mental
Health Services such statistical reports as may be required by
said Director.
SIXTH: This agreemen~ may be cancelled by either party
hereto upon giving thirty (30) days written notice to the other
party.
NOTWITHSTANDING any other pravisions to the contrary, this
agreement wi11 not be effective until such time as the Board of
Supervisors adopts a budget appropriation to cover the cost of
services to be pravided; and until such time as ~he agreement is
approved by the Director o~ the Department of Mental Hygiene.
ZN WITNESS WHEREOF the parties hereto have executed this
Agreement on the day and year first above w~itten.
