WHEREAS, Ordinance 1296 requires in 5ec~tion 2C1 that certain
property owners applying to the County ~or .permits complete a
questionnaire in a form approved by resoiution o~' this Board.
WHEREAS, it is necessary to approve a qusstionnaire form
forthwith to expedite the processing of permi~s by the County.
NOW, THEREFORE, BE IT RESOLVED by this Board that the
Environmental Impact Ques~ annaire attached hereto as Exhibit "A~~
is appraved by this Board for use pursuant to Section 2C of Ordinance
1296.
