NOW, THEREFORE, BE TT RESOLVED that Section 3(c) of Resolution No, 73-138
which reads as follows be deleted:
Seeti.on 3(c), Effective September l, 1973 a supplemeatal
~orm entitled "Persona]. Car ~~iileage Chargeable to Butte County" shall
accampany the regular county claim form. The supplemental form shail
be completed by al1 officers and employees c~aiming private vehicle
m~leage reimbursement. Both the claim and supplemental form wi11 be
submitted to the appropriate department head for apgroval. and for-
waxdzng to the Auditor for payment. Claim forms noC accompanied by
the supplemental form will be seturned ta Che department head for
proper documentation prior to paymente
