Survivors, Disabili'ty~ and ;iealth Insurance) Agreement between ti~e

Wr'IE~tEAS, the ~ ~pTJ~Fix tTF BU~TE , hereina£ter
referred to as "Public Agency", and the State of California enterec into an
agreement on~Septem'ber 15 , ~-959 , providing for the inclusion o_° t,ne
services of elig3ble em~loyees o£ the Public Agency in the Califorr.ia :;tute
Social Security Agreement o~ rar~n 9, ~-951, pus'suant to the provisions of'
Section 218 of the Social. Security Act; and
WHE_RF.4S, coverage o~' such services under the said agreement between the Public ~geucy
and the State oP Ca1.3fornia has been in effect for not less than five fears
from the e~~ective date o£ coverage specified in said agreement, as required
by ~aw fbr termination of coverage; and
Wf~REAS, it is understood that if the said agreement between the Publ.ic Agency and the
State of California, and the Cal3fornia State Social Security~A~reement vith
respec~ ta the Public Agency, are terminated, the Public Agency ma.y not aga3n

enter into an agreement with the State to prov~de for ineZusion of tha serviees
of its employees in the California State Social Security Agreeraent; Elow, there-

the best interest of the Public Agency to terminate said a~ree:r.e:~t uir.n respect

RESOLUED, That the said Board of AdministraCion be, and it is hereby, requested to
take such action as is required to effect termination af tAe California
State Social 5ecurity Agreement of March 9, iq51, with respect to ali
coverage groups of the Public Agency, effettive B~farcl~ 31 , 19~8 ;
and be it further
RESOLVED, 'Fhat the Authorized Agen~ of the Pubtic Agency be and is hereby
directed to forward to the Board of Administratiort a certified copy of
this Resolution.
