WHER~aS, on June 15, 1982 the Board of Supervfsors adopted
Resolutian 82-96 giving notice of intent to form an Adult Day Health Planning
Council. Said Resolutian and proof of publication are attached hereto as
~xhibit I; and
WHEREAS, the Board of Supervisors at their regularly scheduled
meeting on July 20, 1982 formed an Adult Day Health Planning Council. A
copy of the minutes of this meeting and Board actTOn are attached as
Exhibit II; and
WH~REAS, the Board of Supervisors a~ ~heir regu1arly schedu7ed
meeting on September 14, 1982 appointed 21 individuals to serve on the Adult
Day Health Planning Cauncil. A copy of the minutes of this meeting and Board
action are attached as Exhibit III; and
I~HEREAS, the Board of Supervisors desires to delegete to the Area
Agency on Aging responsibiZity for providing s~aff work for the development
of a County Adult Day Health Care Plan and also for providing assistance
with appointments to the P~anning Council.
NOW, THEREFORE, BE IT RESOLVED that pursuant to actions by the
Board of Supervisors on July Z0, 1982 and September 14, 1982 an Adult pay
Hea]th Planning Counc~l is established and 21 indzviduals are appointed to
serve on this Council.
BE IT F~RTHER RESOLVED that the Board of Supervisors delegates
to the Area Agency on Aging responsibility for providing staff work for the
development of a County Adult Day Health Care Plan and also for provfding
assistance with appointments to the County Adu~t Day Health Planning
Counci1; and
Resolution Establishing the Adult Day Health Care Council and
the Appointment of Council P4embers; and Designating the Area
AQencv on Aain4 to Provide Certain Staff Nlork for the Council
BE IT FURTHER RESOLVED that the Chairman of the Saard of
Supervisors is authorized to sign a letter of transmittal ~o the State
Department of Heaith Services requesting approval of the County's Adult
Day Health Planning Cou~cil.
