Houser, Auditor- Controller DATE: March 1 S, 2012 oRO«~~ ~ "~'~~R~~'` SUBJECT: Behavioral Health -Relief of Accountability from Fixed Asset (Damaged Vehicle} The Department of Behavioral Health is requesting Relief of Accountability from fixed asset #26700, Dodge Dakota Quad Cab, VIN #lD7HG38X23S138851,Buffe County ID #1160. The vehicle was stolen then recovered but vandalized. The vehicle had been fully depreciated and had no residual value left. The cost of repairs exceeds the value of the vehicle. There was no evidence that the incident of stolen vehicle was the result of fraud or gross negligence as identified in Government Code Section 29390. If the Board determines that no fraud or gross negligence exists, it would be appropriate for the Board to approve the Department of Behavioral Health's Request for Relief of Accountability of the above mentioned vehicle, when requested by the Department, in accordance with Government Code Section 29390. r DAH/AL cc: Anne Robin, Director of Behavioral Health Amy Wilner, Assistant Director of Behavioral Health rNlichael L.
