(Proposition 65). The report documents information regarding the illegal discharge (or
threatened illegal discharge) of hazardous waste, which could cause substantial injury to
the public health or safety. The report is being submitted on behalf of all designated employees
of the Department of California Highway Patrol.

1 Sequence of Events

3 On March 6, 2018, at approximately 0030 hours, Ms. Caitlin Daniel was driving her 2009 Ford
4 Escape westbound on Skyway. Ms. Daniel allowed the Ford to travel to the left, and collided
5 with a bridge rail over Butte Creek. The Ford continued off the bridge and landed in the water
6 below in Butte Creek. Ms. Daniel self-extricated and left the scene following the collision. The
7 , collision was discovered during daylight and CHP Officer Miguel DeLeon responded to handle
8 the collision investigation.

10 No leakage was initially observed from the vehicle into Butte Creek. The California Department
11 of Fish and Wildlife was notified of the vehicle in the water. California Department of Fish and
12 Wildlife Lieutenant Barry Ceccon, ID 495, responded to the scene to observe any potential
13 damage to the waterway.

15 During vehicle extrication, fuel was observed leaking from the Ford into the water. The leakage
16 was estimated to be less than 16 gallons of gasoline. Because the entire fuel release was in the
17 waterway, Lieutenant Ceccon took incident command of the hazardous material incident and
18 made notification to the California Office of Emergency Services (OES). The Ford was
19 subsequently removed from the creek.

27 Lieutenant Barry Ceccon determined no cleanup was necessary and declared the scene safe at
28 0945 hours.
