The report documents information regarding the illegal discharge (or threatened illegal discharge) of hazardous waste, which could cause substantial injury to the public health or safety. BAXTER, Captain Commander Chico Area Enclosures t6t!,VZQ� Saftty, Senlice, and Security An Internationally Accredited Agen cy NARRATIVE/SUPPLEMENTAL Pae , of 2 DATETII'IE 'CIc NUMBER OFFICER C.D.N NIBER REPORT'NUMBER 04/17/2018 0445 9241 14768 1 Sequence of Events 2 3 On May 17, 2018, at approximately 0445 hours, Mr. Hong Hai was driving his 2009 Freightliner 4 while towing a single flatbed trailer carrying an empty freight cargo container. He was eastbound 5 on Ord Ferry Road east of River Road. Mr. Hai diverted his attention to his GPS device and 6 allowed his big rig to drift to the right. As he drifted to the right, he was just about to enter the 7 Angel Slough Bridge. Mr. Hai's vehicle struck the wood/metal guardrail and concrete footer of 8 the bridge. As the result, the front axle of the big rig was broken which caused the right front tire 9 to crush the vehicles right side fuel tank.- Mr. Hai's big rig at that point became uncontrollable 10 and continued along the guardrail until it toppled over the guardrail and landed on its right side in 11 slough below. The slough consisted of a dry area of vines and trees that separates two 12 waterways. The waterways were not affected by the collision. The collision was called in by off 13 duty CHP Officer Kevin Baxter, who remained on scene until CHP Officer Ron Simmons 14 responded to handle the collision investigation. 15 16 The result of the crushed fuel tank resulted in approximately 50-75 gallons of diesel fuel to dump 17 out into the dirt under the fuel tank. California Department of Fish and Wildlife Officer John 18 Amis, Office of Spill Prevention and Response,was summoned to the scene to observe any 19 potential damage to the waterway. Butte County Road Department was also called to the scene 20 along with Butte County Department of Public Health Hazardous Material Specialist Dan 21 Holochwost. 22 23 Evacuation Details 24 25 No evacuation measures were necessary. 26 27 Cleanup Actions 28 29 NRC Environmental Specialists were called to the scene for cleanup by Mr. Hai and his 30 insurance agency. The closure remained in effect 5 until May 18, 2018 at approximately 1500 hours. 6 7 Environmental Impact 8 9 Environmental impact was low and NRC is working with Butte County Public Health Officials 10 until all contaminated soil is removed and no contaminants are detected. 11 12 Actions of other agencies 13 14 CDF Hazmat Officials notified OES of the spill and were given an OES control number of 19- 15 3194. 16 17 Other information 18 19 A Site Safety Plan (SSP) and an Incident Action Plan (IAP)was prepared by CHP Sergeant Dan 20 Canalia and CDF Hazmat Specialist Chad Porter, who was also deemed as Safety Officer.
