Report # Page # C 10-30-2008 1433 173 3 1 SEQUENCE OF EVENTS 2 On October 30, 2008, at approximately 1433 hours, a 1996 Peterbuilt Tractor with 2 3 dump trailers, being driven by Sukdhip Singh of Yuba City, was northbound on SR 99 in 4 the #2 lane at 50 mph approaching the E. First Avenue off-ramp when he observed 5 traffic slowing ahead. The driver applied his vehicles brakes rapidly but was unable to 6 slow quickly and turned his vehicle sharply to the right to avoid a collision with stopping 7 traffic. His vehicle proceeded to veer right and the tractor jackknifed, crossing the right 8 shoulder and slid partially over the embankment. As the vehicle jackknifed, the front 9 dump trailer pierced and ruptured the right fuel tank, spilling approximately 10 gallons of 10 diesel fuel on to the dirt embankment area. 11 12 During the vehicle towing process when the tractor was being dragged back to the 13 roadway from the embankment, the vehicle fuel tank's crossover hose was torn from 14 one of the fuel tanks, spilling an additional 20 gallons of'diesel fuel on to the asphalt 15 shoulder. The diesel fuel tank crossover hose was quickly plugged and the spilled fuel 16 was quickly contained with "magic sorb" absorbent and limited to the shoulder area. 17 18 ROAD CLOSURES 19 The SR 99 northbound #),lane was closed at the collision scene for approximately 3 20 hours for accident investigation, vehicle removal and mitigation. 21 22 EVACUATION DETAILS 23 None. 24 25 ENVIRONMENTAL IMPACT 26 None. 27 28 CLEAN-UP ACTIONS 29 NRC Environmental Services responded to transfer the remaining diesel fuel from the 30 fuel tanks and for removal of the contaminated embankment soil and absorbent. 31 32 ACTIONS OF OTHER AGENCIES 33 Butte County Public Health was notified but did not respond. CalTrans responded for 34 traffic control and clean-up. Chico Fire Department responded with their HazMat unit 35 and fire suppression teams.
