County Service Area No. 99 loan - a resolution has been prepared to loan CSA No. 99 $330 from the CSA revolving loan fund. General Cellular Corporation cnmmunicatians vault lease -- a contract has been prepared with the organization to lease space in the County's 479 E. Fork Avenue, Chico communication site in the revenue amount of $150 per month covering the period December 1, 1.988 through November 30, 1993 - action requested - APPROVE CONTRACT AND AUTHORIZE THE CHAIRMAN TO SIGN. 21. Jere Bolster contract amendment - an amendment has been prepared providing for a one year extension through February 28, 1990 with all other provisions remaining intact - action requested - AUTHORIZE THE CHIEF ADMINISTRATIVE OFFICER TO SIGN THE AMENDED AGREEMENT. 22. Northern California Research Associates contract - a proposal has been received from the organization to conduct a study of the fees and services in the County Clerk, Recorder, and Elections Offices to determine the appropriate License charges and fees for services provided to the public. The contract is in the amount of S9, 000 with funding coming from the Recorder' s Systems Trust Fund - action requested - APPROVE CONTRACT AND AUTHORIZE THE CHAIRMAN TO SIGN. 23. Property tax exchange agreement - Walnut Investors Annexation - a resolution and agreement have been prepared covering the Thermalito Irrigation District annexation of Walnut Investors with no actual property tax being transferred - action requested - ADOPT RESOLUTION 89-023 APPROVING THE AGREEMENT AND AUTHORIZE THE CHAIRMAN TO SIGN. 24. Vital Record Improvement Project agreement - an agreement has been prepared with the State Department of Health Services in the amount of 515, 000 state money to purchase computer hardware and software to be utilized in improving the collection and handling of birth records within the County - action requested ~- APPROVE THE AGREEMENT AND AUTHORIZE THE CHAIRMAN TO SIGN. Page 56 February 28, 1989 Februar 28 1989 25. 1988-89 County Health Services agreement - an agreement has been prepared with the State Department of Health Services in the amount of $1,500,623 covering a portion of the costs of County health services during the current fiscal year in accordance with Welfare and Institutions Code Sections 16700-16713 - action requested - AEPROVE THE AGREEMENT AND AUTHORIZE THE CHAIRMAN TO SIGN. 25. Karen Knight amendment - an amendment has bean prepared to the agreement with Karen Knight providing counseling services to child protective cases wherein the hourly rate is changing to $50 per hour effective January 1, 1989 ~ action requested - APPROVE AMENDMENT AND AUTHORIZE THE CHAIRMAN TO SIGN. 27. Linda Wallace amendment - an amendment has been prepared to the agreement with Linda Wallace providing counseling services to child protective cases wherein the hourly rate is changing to $50 per hour effective January 1, 1989 - action requested - APPROVE AMENDMENT AND AUTHORIZE THE CHAIRMAN TO SIGN. 28. Janice O'Donnell amendment - an amendment has been prepared to the agreement with Janice O'Donnell providing counseling services to child protective eases wherein the hourly rate is changing to X50 per hour effective January 1, 1989 - action requested - APPROVE AMENDMENT AND AUTHORIZE THE CHAIRMAN TO SIGN. 29.
