Pledge of allegiance to the Flag of the United States of America. The Board's minutes of May 21, 1991 do not officially reflect the tragic and sudden death of Supervisor Kevin Campbell., nor the grief and sorrow that all of us feel and extend to his family, his constitutents, and his friends. We, as a County, and as a Board, extend condolences to Mrs. Alice Campbell and her sons, Susan Darrough and their sons, and his companion Robin Jossel and her children. Community Action Agency - action requested - APPOINT FRANK O. FENLEY TO FILL AN UNEXPIRED TERM TO EXPIRE DECEMBER 31, 1992. 5. Agricultural Commissioner - Board authorization is requested to purchase one electric typewriter in the maximum amount of $515, and one Televidea 965 computer terminal with keyboard in the maximum amount of $440 from existing fixed asset funds - action requested - APPROVE PURCHASE OF FIXED ASSET ITEMS. 6. Agricultural Commissioner - Board autharization is requested to purchase an axle, hub, brakes and electric brake control unit for a Weights and Measures' two-wheel weight trailer (asset no. 204) in the maximum amount of $400 from existing fixed asset funds - action requested - APPROVE PURCHASE OF FIXED ASSET ITEM. 7. Air Pollution Control Officer - Board authorization is requested for the purchase of an office chair in the maximum amount of $316.43. Budget transfer B- 225 has been prepared for the necessary appropriation - action requested - APPROVE PURCHASE OF FIXED ASSET ITEM. 8. County Fire Warden - Board autharization is requested for the purchase of a heavy duty transportation cart, a transmission jack, and a porta power jack in the maximum amount of $3,372.22 to outfit County mechanics' service trucks. Budget transfer B-213 has been prepared for the necessary appropriation - action requested - APPROVE PURCHASE OF FIXED ASSET ITEMS. 9. County Fire Warden - Board authorization is requested to purchase 15 self contained breathing apparatus air cylinders for use by firefighters in hazardous environments in the maximum amount of $7,000. Conveyance of County property - Board authorization is requested for the donation of a 1960 IH-Van Pelt fire engine (County No. 959), license no. E115505, to the State of California, Office of Emergency Services for refurbishment under the State Assistance for Fire Equipment Act (SAFE) - action requested - AUTHORIZE PUBLICATION OF NOTICE OF INTENTION TO CONVEY COUNTY PROPERTY PURSUANT TO GOVERNMENT CODE SECTIONS 253b5 AND 6061. 11. Award of bad 76-91 - copier control system - the Purchasing Department received and opened bads for a copier control system to track usage on various department accounts. The bids have been evaluated and the lowest bid meeting specifications is San Sierra Business Systems of Chico in the amount of $10,604 plus tax - action requested - AWARD BID. 12. Buildings and Grounds - Board authorization is requested to purchase two replacement pickup trucks at an estimated cost of $25, 000 total, and a $15, 000 man-lift (safety equipment) to be installed on an electricians' van. Budget transfer B-226 has been prepared for the necessary appropriation - action requested - AUTHORIZE PURCHASE OF FIXED ASSET ITEMS. 13. Mental Health Department - Board authorization is requested to purchase two nine passenger vans for $15,000 each, one faun door sedan for $13,000, one mini-van for $13,000, miscellaneous furniture in the maximum amount for $2,097, three IBM model 55 computers for $7,500, computer software for $2,250, a FAX machine far $750, and a Hewlett Packard LaserJet TTI printer for $1,500 from non-local funds. Budget transfer B-220 has been prepared for the necessary appropriation - action requested - APPROVE PURCHASE OF FIXED ASSET ITEMS, 14. South Butte County Municipal Court - Board authorization is requested to purchase five typewriters, WordPerfect computer software, assistive listening device receiver charger, six assistive listening device receivers, U-shaped desk system, three Data General terminals with keyboards and cables, and one paper shredder in the maximum amount of $23,500. The County will be reimbursed $72 per initial inspection conducted not to exceed $7,416 - action requested - APPROVE AGREEMENT AND AUTHORIZE THE DIRECTOR OF WEIGHTS AND MEASURES TO SIGN. 23. City of Biggs/County of Butte - proposed is a renewal of the cooperative agreement to provide fire protection, rescue and emergency medical services in the maximum amount of $11,810 per year commencing July 1, 1991 through June 30, 1992 - action requested - APPROVE AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN SUBJECT TO APPROVAL OF COUNTY COUNSEL. 24. DWC Consulting Company, Inc. - an agreement has been prepared in an amount not to exceed $13,600 to provide technical project management services and related testing for the Probation building crawlspace asbestos elimination project -action requested - APPROVE AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN SUBJECT TO APPROVAL OF COUNTY COUNSEL AND THE AUDITOR-CONTROLLER. 25. Tehama Alcohol Recovery Center, Inc. - the proposed amendment to the contract for the provision of alcohol and drug intoxication and recovery services for the period July 1, 1990 through June 30, 1991 increases the maximum contract amount to $155,805 (an additional $5,423 of drug funds for a subtotal of $33,423, with alcohol funds remaining at $122,382) - action requested - APPROVE CONTRACT AMENDMENT AND AUTHORIZE THE CHAIR TO SIGN. 26.
