Paul and N. Joyce Lowrey -proposed is renewal of a contract for the provision of coordination and supervision services to the H.E.R.E./Youth Services Unit of Alcohol and Drug Services for the period July 1, 1991 through December 31, 1991 in the maximum amount of $2 4 , 3 4 0 - action requested - APPROVE CONTRACT AND AUTHORIZER THE CHAIR TO SIGN. 16. Kate McCracken - proposed is renewal of an agreement for the provision of clinical supervision to the Youth Services Unit of Alcohol and Drug Services for the period July 1, 1991 through December 31, 1991 in the maximum amount of $20,890 - action requested - APPROVE AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN. Vital. Records Improvement Project - all conditions of the contract between the State Department of Health Services and the County have been met and the County has been fully reimbursed for the services provided - action requested - APPROVE CONTRACTOR'S RELEASE AND AUTHORIZE THE CHAIR TO SIGN SUBJECT TO APPROVAL BY COUNTY COUNSEL AND THE AUDITOR- CONTROLLER. 19. Amendment no. 1 to agreement for transportation services on Butte County Transit for Enloe Hospital - proposed is an agreement with Enloe Hospital to continue providing 2.5 hours of daily service between Paradise and Chico for fiscal year 1991/92 - APPROVE AMENDMENT AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN. 20. Catalyst Women's Advocates, Inc. - proposed is amendment no. 12 to an agreement relating to operation of a domestic violence program in Butte County which extends the term of the agreement from July 1, 1991 to June 30, 1992 at the cost of $22,800 - action requested -- APPROVE AGREEMENT AMENDMENT AND AUTHORIZE THE CHAIR TO SIGN. 21. Child abuse therapy - proposed is renewal of counseling contracts for abused children between the Welfare Department (CPS) and Family Services Association ($9,950) from May 1, 1991 to December 31, 1991 and Suzie Heald ($4,000) from April 1, 1991 to December 31, 1991 Family Services Association - APPROVE CONTRACTS AND AUTHORIZE THE CHAIR TO SIGN. 22.
