Award of bid (13-93) - janitorial service - bids have been opened for providing janitorial services of selected leased buildings with the low bidder being Fresh & Clean Janitorial Service, Oroville in the amount of $1,407 per month - action requested - AWARD BID, APPROVE AGREEMENT, AND AUTHORIZE THE CHAIR TO SIGN. 2. County of Butte/Durham Recreation and Park District - proposed is an agreement to lease the Durham Memorial Hall for a period of one year - action requested - APPROVE LEASE AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN. 3. National Homecare Systems agreement - In-Home Supportive Services (IHSS) - the current contract is a third year extension of a two year contract approved in 1990. Based upon the maximum hours limitation of the contract, the Welfare Department has determined that the contract will need to be terminated on or about November 18, 1992 in order to keep the County from being at risk for funding over the limitation - action requested - APPROVE NOTICE OF TERMINATION. Award of bids (16-93) - the Purchasing Office received and opened bids for two compact 4x4 pickup trucks and one 1/2 ton 4x2 pickup cab and chassis for the Public Works Department. The bids have been evaluated and the lowest bids meeting specifications are Wittmeier Chevrolet-Geo, Chico in the amount of $17,621 each for the two pickups and $13,912 for the pickup cab and chassis plus tax - action requested - AWARD BIDS. 11. Notice of completion - Juvenile Hall storeroom {bid 57-92) - the project has been completed in accordance with the plans and specifications, as amended - action requested - APPROVE NOTICE OF COMPLETION AND AUTHORIZE THE CHAIR TO SIGN. 12. Butte County Library Corporation - final audit report - Matson and Isom Accountancy Corporation submits the audit report through June 30, 1992 - action requested - ACCEPT FOR INFORMATION. 13. Connerly and Associates, Inc. - proposed is an agreement to administer the 1992 Community Development Block Grant Program (CDBG) for housing rehabilitation in Stirling City, Thermalito and Palermo for 24 months from the date of execution of the agreement. The maximum funds available under this contract shall not exceed $79,980 (grant funding) - action requested - APPROVE AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN. Introduction of a salary ordinance amendment implementing a reallocation of Department of Development Services - Building Inspection staff in order to more accurately reflect workload as well as allow flexibility of current staff functions. No new or additional positions are being requested - action requested - WAIVE READING OF THE ORDINANCE. 15. Adoption of a salary ordinance amendment implementing an organizational change in the current structure of road maintenance crews, and correcting an administrative error in staffing levels in the ordinance adopted by the Board on January 4, 1992 for the clean-up and appeals hearing process of the Nash compensation study. The proposed ordinance does not add any new positions - action requested - ADOPT ORDINANCE 3031 AND AUTHORIZE THE CHAIR TO SIGN. 16. Resolution approving termination of the site lease and facility lease by and between the County and the Butte County Library Corporation and assumption of liability, assets and property - action requested - ADOPT RESOLUTION 92-131 AND AUTHORIZE THE CHAIR TO SIGN. 17. Page 318 October 6, 1992 92-378 October 6, 1992 18. 1992/93 State Department of Food and Agriculture pest detection contract no. 92-0199 - exhibits B and C to contract, as approved by the Board on June 9, 1992, have been revised to reflect a reduced number of traps and a reduction in the contract amount from $27,518 to $23,390 (State funds) - action requested - APPROVE CONTRACT AND AUTHORIZE THE CHAIR TO SIGN. 19. 1992/93 State Department of Pesticide Regulation pesticide residue contract no. 92-0374 - the proposed contract is in the maximum amount of $3,600 commencing July 1, 1992 through June 30, 1993 - action requested - APPROVE CONTRACT AND AUTHORIZE THE CHAIR TO SIGN. 20. County Service Area (CSA) No. 37 (Gridley-Biggs Ambulance) - proposals (RFP) were solicited in August, 1992 for the provision of ambulance services for CSA No. 37 (Biggs, Gridley, Richvale areas). The CSA No. 37 Ambulance Service Advisory Committee met, reviewed the submissions, and recommends N. T. Enloe Memorial Hospital, Chico. The term of the contract is October 1, 1992 through September 30, 1995 in the maximum amount of $60,000 per year - action requested - APPROVE AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN SUBJECT TO APPROVAL OF COUNTY COUNSEL AND THE AUDITOR-CONTROLLER. 21. Community Action Agency/County of Butte - the proposed agreement provides for the removal of nuisance debris at 3305 Spencer Street, Oroville (AP 35-111-014, declared a public nuisance by the Board on August 4, 1992). The agency will provide the County with an itemized billing for services rendered in the course of the nuisance debris removal to be paid from a recovery on a lien against the owner's proprietary interest - action requested - APPROVE AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN. 22. Butte County Office of Education (BCOE) - the proposed agreement provides for school-based prevention and coordination services for youth at risk of drug and alcohol abuse in Butte County. The term of the agreement is July 1, 1992 through June 30, 1993 in the maximum amount of $57,313 (Federal/State) - action requested - APPROVE AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN.
