County Service Area No. 145 (Chico Gardens) - the Local Agency Formation Commission has approved the formation of the county service area to provide for street lighting and storm drainage maintenance pursuant to Government Code Section 25210.14 - action requested - ADOPT RESOLUTION 92-136 AND AUTHORIZE THE CHAIR TO SIGN. 11. Used forklift - the Public Works Department has completed their evaluation of numerous used forklifts to best meet their needs and is recommending the sole source purchase of a 1990 Case, model 486E forklift from Hertz Equipment Rental, Sacramento, in the amount of $24,100 plus tax (road fund) - action requested - APPROVE PURCHASE OF FIXED ASSET ITEM. 12. Sheriff-Coroner - portable helicopter fuel tank - Board authorization is requested for the acquisition of a portable, 500 gallon fuel tank in the maximum amount of $750 (department budget). The tank will be parked on the Sheriff's helipad or towed to the site of extended helicopter operations, i.e., search and rescue, marijuana eradication, fires, etc., and will provide a substantial savings of flight time and fuel costs by not having to fly to Chico Airport for fuel. Budget transfer B-19 has been prepared to provide the necessary appropriation - action requested - APPROVE PURCHASE OF FIXED ASSET ITEM. 13. Investment information report - the Treasurer/Tax Collector submits the September 30, 1992 report in accordance with AB 1073 and the County's investment policy - action requested - ACCEPT FOR INFORMATION. Payment of time expired warrant - Laurie McErquiaga - a request has been received for payment of a time expired warrant (No. 021556) issued on June 30, 1989 in the amount of $100 for an IRS refund from the District Attorney's Family Support Division - action requested - AUTHORIZE THE AUDITOR-CONTROLLER TO REISSUE TIME EXPIRED WARRANT. 15. Adoption of a salary ordinance amendment implementing a reallocation of Department of Development Services - Building Inspection staff in order to more accurately reflect workload as well as allow flexibility of current staff functions. No new or additional positions are being requested - action requested - ADOPT ORDINANCE 3032 AND AUTHORIZE THE CHAIR TO SIGN. 16. Adoption of an ordinance adding Chapter 31.1 to the Butte County Code establishing a Solid Waste Permit Enforcement Hearing Panel which will comply with the Public Resources Code requirement to appoint a hearing panel. The appointment of a hearing panel partially fills Solid Waste Enforcement Agency redesignation requirements of Environmental Health - action requested - ADOPT ORDINANCE 3033 AND AUTHORIZE THE CHAIR TO SIGN. 17. Program supplement no. 012 to master agreement no. SLTPP-5912 - reimbursement of funds for qualified projects under the State-Local Transportation Partnership Proqram (SLTPP), farmerly SB140/SB300, requires execution of a program supplement to master agreement no. SLTPP-5912 - action requested - APPROVE PROGRAM SUPPLEMENT NO. 012; ADOPT RESOLUTION 92-137; AND AUTHORIZE THE CHAIR TO SIGN. 18. 1992/93 Primary Intervention Program (PIP) - contract no. 92-72049 has been prepared with the State Department of Mental Health in the amount of $38,985 (State funds) to continue a Primary Intervention Program in the Chico Unified School District covering the period July 1, 1992 throuqh June 30, 1993 - action requested - APPROVE CONTRACT AND AUTHORIZE THE CHAZR TO SIGN. Chico Unified School District - the proposed contract with the County's Mental Health Department has been prepared to continue the Primary Intervention Program at three elementary school sites within the District. The term of the contract is July l, 1992 through June 30, 1993 in the maximum amount of $38,985 (State funds) - action requested - APPROVE AGREEMENT AND AUTHORIZE THE CHAIR TO SIGN. 20. 1991/92 Negotiated Net Amount (NNA) contract - proposed is an amendment to the contract (contract no. NET04-91) which provides (SB 1528) for the carryover of funds in 1992/93 - action requested - APPROVE AMENDMENT AND AUTHORIZE THE CHAIR TO SIGN. 21. 1992/93 application for Rural Health Services - 100 percent State funds will be available to the County to help cover the costs of noncompensated hospital and physician medical care and to cover selected other medical activities - action requested - APPROVE APPLICATION AND AUTHORIZE THE CHAIR TO SIGN. 22. Agreement regarding sewage disposal easements for Rocky Bluffs Subdivision, Phase I, AP 11-42-07, twenty lots and common area located on the north side of Skyway, east of Skansen Subdivision, east Chico area, and County Service Area 21 facilities - the County through the proposed agreement, requires a contract with the Rocky Bluffs Property Owners Association, to be primarily responsible for the operation and maintenance of its sewage disposal facilities within County Service Area 21.
