Pursuant to CDPH All Facilities Letter 10-28-dated November 12, 2010, this
letter is to notify you that effective November i, 2019, Enloe Medical Center
will no longer be partnering to provide primary care services at the following
location:
Gridley Medical Group-Family Practice Clinic
225 Spruce Road
Gridley, Ca 95948
Gridley Medical Group is currently working on a plan to continue operations
at this site and we anticipate no interruption of services for patients who are
being care for at this location.
Attached you will find the letter submitted to CDPH by our CEO. Please
