WHEREAS, the Policy Council for the Butte County Health Assessment and Improvement Plan (Council) was
formed in 2014 in response to hundreds of State and local agency representatives and community members joining the
Butte County Public Health Department in forming the Together We Can! Healthy Living in Butte County partnership to
evaluate the County's health status and prioritize areas for improvement; and
WHEREAS, the purpose of the Council was to direct the strategy for the Community Health Assessment (CHA) and
the Community Health Improvement Plan (CHIP) and had final approval before the CHA and CHIP were widely
disseminated; and
WHEREAS, the Council has completed the work it was tasked with.
NOW, THEREFORE, BE IT RESOLVED that the Butte County Board of Supervisors hereby disbands the Policy Council
for the Butte County Health Assessment and Improvement Plan.
