STATE DEPARTMENT OF HEALTH CARE SERVICES AND
BUTTE COUNTY DEPARTMENT OF BEHAVIORAL HEALTH

WHEREAS, Butte County Department of Behavioral Health requires authorization from the Butte County Board of
Supervisors to enter into the State Department of Health Care Services (DHCS) Substance Use Disorder Agreement for
Substance Use Prevention and Treatment Block Grants for FY 2017-2018 through FY 2019-2020; and
WHEREAS, DHCS requires the Butte County Board of Supervisors to authorize Scott Kennelly, Director, Butte
County Behavioral Health to sign the agreement and all amendments for FY 2017-2018 through FY 2019-2020; and
NOW, THEREFORE, BE IT RESOLVED that the Butte County Board of Supervisors hereby authorizes the Butte
County Department of Behavioral Health to accept this agreement with the State Department of Health Care Services for
a multi-year total of $4,947,043 and to accept above described provisions of this agreement..
