Due to the size of the campus and duration of inspection I was unaccompanied, however contact was made prior to inspection with Mark Allen, Maintenance Lead, 530-624-4199. Inspection The inspection consisted of the entire campus including; Public Safety Training Grounds (PSTG), Facilities Planning Management (FPM), Recycling Center, Physical Education (GYM), Mens Locker Room (MLR), Womens Locker Rooms (WLR), Arts (ARTS), Student Health Clinic (SHC), Swing Space A (SSA), Swing Space C (SSC), Swing Space F (SSF), Swing Space G (SSG), Regional Testing Center (RTC), Business Education (BE), Student Administration Services (SAS), Campus Center (CC), Allied Health/Public Service Center (AHPS), Library (LB), Media Center (MC), Learning Resource Center (LRC), Life Science (LS), Automotive Technology (AT), Physical Science (PS), Welding Manufacturing (WM), Ag Storage, Mechanized Ag (MA), Ag Repair Shop, Horticulture Storage, Green Houses, and Environmental Horticulture (EH). The Child Development Center was previously inspected. During the inspection the following deficiencies were noted. Deficiencies PSTC- Fire Grounds Training Building Remove items blocking Air Compressor Electrical Disconnect LDPG#6, CFC 605.3 Remove storage (tires)from Fire Sprinkler Riser Room, CFC 315.3.3, 509.2 PSTC- Fire Grounds Training Building cont. Fire Alarm Report from 12-28-2018 by Voltage Specialist states that white board is covering annunciator. Was white board removed or annunciator relocated? Fire Damper Inspection Report from 11/13/2016 by Voltage Specialist states multiple dampers failed to close due to a bad actuator. Have repairs been made? Gym 123 Label Fire Department Connection. A metal sign with letters no less 2" high, and a contrasting background. Sign should read "FDC" and denote the building the connection serves. CFC 912.5 MLR 114 LPG container stored inside office. Remove container. CFC 6103.2.1 ARTS Building Label Fire Department Connection. A metal sign with letters no less 2" high, and a contrasting background. Sign should read "FDC" and denote the building the connection serves. CFC 912.5. Remove blue tape on sprinkler head escutcheons at northeast exterior corner of building adjacent to ARTS 138. Fire Damper Inspection Report from 12/22/2016 by Voltage Specialist states; FSD 87 key is broken off in switch. Switch need replaced; FSD 78 is STUCK CLOSED, FSD 41 switch did not close damper, FSD 4 switch had no light and did not close damper; next to FSD87 there is an unused damper test switch. Have repairs been made? ARTS 105 Perform general housekeeping, correct storage, CCR T19, Div.1 §3.08 ARTS 162 AN Loft Remove items blocking Handicapped Elevator, CFC 1003.6, 1031.2 ARTS 166 At door 166-1, adjust door so that it opens smoothly, and does not contact floor. CFC 1031.2 ARTS 165 Hall Remove rolling chair and riser racks from egress corridor. CFC 1031 and CCR T19, Div.1 §3.11 Remove recycling can from under fire door. CFC 703.2 ARTS 105 Perform general housekeeping, correct storage. CCR T19, Div.1 §3.08 ARTS 252 Reinstall electrical panel cover on panel H2A. CFC 605.6 Regional Test Center(RTC) South Door, Emergency Exit Sign non-operational, CFC 1008& 1104.5 RTC Room 103, Hall, Emergency Exit Light non-operational, CFC 1008 & 1104.5 SAS Label Fire Department Connection. CFC 912.5 CC Fire Alarm Report from 12/28/2018 by Voltage Specialist states battery was not dated. Has battery been replaced? CFC 315.3.3, 509.2 AHPS 173 Clean Agent System, provide semi-annual testing and maintenance report, CCR T19 Div. 1 § 904 AHPS North Fire Alarm Report from 1/13/2019 by Voltage Specialist states system duct detector failed to trigger alarm? Has detector been replaced? Fire Damper Inspection Report from 12/22/2016 by Voltage Specialist states several dampers failed. No note/correction page attached to report. Have repairs been made? BE Fire Alarm Report from 12/26/2018 by Voltage Specialist states system HVAC does not have duct detection device? Has detection been installed?Additionally, southwest pull station was noted as sticking. Has device been repaired or replaced? TE Provide current copy of annual testing and maintenance for FM 200 Extinguishing System, last was 3-28-2018. CDC Fire Alarm Report from 12/28/2018 by Voltage Specialist states battery was not dated. Has battery been replaced? LS Fire Damper Inspection Report from 11/02/2016 by Voltage Specialist states several dampers failed to close when Fire Alarm was activated. Have repairs been made? LRC Fire Damper Inspection Report from 12/02/2016 by Voltage Specialist states damper 12-6 failed to close, and had a bad actuator. Have repairs been made? Fire Alarm Systems-Annual testing and maintenance reports for SAS,TE, LRC as they were not included in reports provided. Fire Dampers-Annual testing and maintenance reports for AHPS South as they were not included in reports provided. Overhead Fire Doors and Curtains- Provide documentation of annual testing and maintenance.These operational tests can be performed by Butte College Maintenance personnel.
