Eleventh Independent Consultant’s Safety Inspection Report recommendations for Sly Creek Dam
Dear Mr. Blackett,
This letter serves as a response to your letter dated October 1, 2020 requesting a status update on the
independent consultant’s recommendation to the STID and DSSMP with a post-earthquake analysis of the Ogee
spillway at Sly Creek Dam. Please accept my apologies for not responding sooner, but the communication was
not received directly by either the General Manager or myself, but found inadvertently in the e-library.
To date, this Agency has not received follow-up correspondence or acceptance of the June 2019 Phase 3 Spillway
Assessment for Sly Creek Dam, compiled by McMillen Jacobs and Associates. This assessment includes a
Technical Memorandum summarizing Sly Creek Dam Spillway Seismic Stability Analysis, which addresses the
2017 Part 12D Inspection recommendations. Due to staff turnover, it is possible that the report was not properly
submitted to you, therefore I am re-submitting this phase of the spillway assessments for your review.
With regard to any other outstanding STID and DSSMP updates, we await your formal notice of approval of the
Phase 3 Spillway Inspection Report, and will submit a plan and schedule for submission of further updates upon
receipt.
