D If the application is signed by an agent for the owner, an agent authorization form must be
submitted along with the application. The application cannot be accepted unless signed by
the owner or authorized agent.·
4.D D A signed agreement for Bu�e County Archaeological Resources Project Review by the n/a per Mark Michelena Northeast Information Center (NEIC) accompanied by its required topographic map and a
check in the amount of$75.00 made out to "NEIC". Alternatively, you may submit proof
of a previous Archaeological Resources Project Review.

at the time of application. Pleas� be prepared to fill in the exact amount required for
application at the time of submittal. Retainer fees for publication noticing as well as fees for

County Treasurer". If the check issuer is not available for submittal of the application, the
check issuer may write, below the line for the written amount of the check, "check amount
not to exceed $::XXX.XXX" , so that the exact amount required can be filled in at the time of
application submittal.

SITE PLAN: Three (3) copies of a detailed site plan drawn to standard engineering
scale ( e.g., 1" = 20'). Additional copies of the map shall be made available to
county departments upon request. All maps shall be drawn on uniform size sheets
no less than 18" X 26". Include one copy of the map on an 11 "xl 7" sheet. In
addition, a digital copy of the map and aerial photographs are encouraged. The
detailed man must include:
Name and business address of the applicant, engineer and/or person who prepared
the maE.
Assessor Parcel Number(s); street address (if available); and/or Township, Range
and Section numbers.
Property lines, lot dimensions and any applicable easements of record or proposed.
City and special district boundaries (if applicable), existing and proposed general
plan and zoning designations for the property and surrounding properties within 300
feet.
Dimensioned locations of existing and proposed public and private improvements
on the property.(including, but not limited to, buildings, roads, driveways, parking
areas, wells, septic tanks, sewer lines, leach fields, utilities, storm drainage systems
and street lights.)
Location of all water features including, but not limited to, natural and human-made
drainage courses, irrigation canals, ponds, creeks, sloughs; topographic features
including, but not limited to, buttes, slopes> 30%, rock out-croppings; and existing
ve�etation, such as oak trees greater than S" in diameter and agricultural crops.
Distances between any significant natural and constructed features of the property,
such as streets, access roads, streams, rock outcroppings, major tree stands, storm
drains, bodies of water, railroads, and their relationship to the property lines.
For vacant or substantially undeveloped land, show contours at an appropriate
vertical scale.
Vicinity map indicating the location of the land in relation to the nearest major
roads and/or significant topographic features in the surrounding area or region.
Indicate scale and include a north arrow oriented to the top of the sheet.
All plans must be clear and legible.

Additional information may be required in order to clarify, amplify, correct or otherwise supplement the above
submittal information or to complete any required environmental review documents, as deemed necessary by the
Department of Development Services, Public Works, Environmental Health Division, Butte County Fire
Department, or Agriculture Commissioner.
When a project is determined to require an environmental assessment, applicant shall be provided an addendum
to the project application for review/signature to authorize work to continue on the project. Applicant will
remain responsible for payment of fees related to the processing of the project application(s), and additionally for
all costs related to the development and dissemination of a Request for Proposal, review of proposals, and
County's selection of a consultant to perform the required work. Once the consultant has been selected, the
applicant shall again authorize the work on the project to proceed with responsibility for payment of
Development Services costs to develop a contract with consultant, and ongoing programmatic and fiscal
review/monitoring of the contract. Along with a retainer for the cost of the consultant contract, applicant shall
also pay a initial retainer equal to 30% of the contract maximum, which shall be replenished by the applicant as
needed.
Additional fees may be collected for the California Department of Fish and Game at the time of recording the
Notice of Determination as required by Public Resource Code (PRC) section 21089 (b).
