Re: Eleventh Lost Creek Dam
Dear Mr. Moseley:
This is in response to your letter dated December 13, 2017 that submitted the 11th
and Potential
Failure Mode Analysis (PFMA) Addendum Report for Lost Creek Dam, which is part of
the South Feather Hydroelectric Project, FERC No. 2088. We have reviewed the report,
and we have the following comments:
1. The categories of the Potential Failure Modes No. 1, 2, 3, 4, and 5 should be
revisited during the next PFMA review session due to the recent modifications of
the dam.
2. As indicated in our May 5, 2017 Part 12 reminder letter, SFWPA is reminded that a
plan and schedule to address the independent consultant recommendations was due
not later than 60 days after the filing of the Part 12 report. Please submit an
updated plan and schedule to address all of the independent consultant
recommendations including the recommendations that have been completed.

Our review of the Part 12D Report concludes that the scope and content of the
report satisfies the requirements of Part 12, Subpart D, §12.37 of Title 18 of the Code of
Federal Regulations. The Part 12D Report was found to be prepared in general
accordance with the Engineering Guidelines Chapter 14.
Within 30 days of the date of this letter, please provide a plan and schedule to
address our comments. We appreciate your cooperation in this aspect of the
